2型糖尿病合并冠心病危险因素及冠状动脉特点分析

摘　　要：目的研究2型糖尿病合并冠心病患者的临床特点,为进一步指导治疗提供依据。方法对50例糖尿病合并冠心病患者、50例单纯冠心病患者、单纯糖尿病患者50例及50名健康对照者的性别、年龄、病程、体质量指数(BMI)、血压、空腹血糖、糖化血红蛋白(HbA1c)、血脂、空腹胰岛素等指标进行对照,并将糖尿病合并冠心病患者冠状动脉病情状况与单纯冠心病患者进行对比。结果糖尿病合并冠心病组和单纯冠心病组与对照组相比,BMI、血压、甘油三酯(TG)、总胆固醇(TC)、低密度脂蛋白胆固醇(LDL)、高密度脂蛋白胆固醇(HDL)存在差别,糖尿病合并冠心病组空腹血糖、HbA1c、胰岛素水平、TG均较单纯冠心病组及对照组高,糖尿病合并冠心病组较单纯冠心病组冠状动脉病变更加广泛,狭窄程度更重。结论血压、TG、TC、LDL、HDL、BMI、HbA1c、空腹胰岛素水平是2型糖尿病患者合并冠心病的危险因素,2型糖尿病患者合并冠心病冠状动脉病变更加广泛,狭窄程度更为严重。Objective To make a research of clinical characters of type 2diabetes patients with coronary heart disease（CHD）and to guide the further treatment for such patients.Methods Four groups are selected for research.Group 1is composed of 50 diabetes patients with CHD.Group 2is composed of 50 pure CHD patients.Group 3is composed of pure diabetes patients.Group 4is composed of 50 health candidates.Many aspects are compared including gender,age,illness course,BMI,BP,fast blood glucose and fast insulin.Analysing is made by comparing group 1and group 3.Results Both group 1and group 3have statistic difference with group 4in the aspects of BMI,BP,TG,TC,LDL and HDL.Some data of group 1are higher than those of group 3and group 4including fast blood glucose,HbA1 c,insulin level and TG.The narrow situation of coronary artery in group 1is worse than that of group 3including size and extent.Conclusion The risk factors for type 2diabetes with CHD included BP,TG,TC,LDL,HDL,BMI,HbA1 c and fast insulin.When combined with CHD in type 2diabetes patients,the pathological changes of coronary artery are more extensive and worse.
