降压药联合他汀类药物对高血压患者血压变异性影响的Meta分析

摘　　要：目的探讨降压药联合他汀类药物对高血压患者血压变异性(BPV)的影响。方法检索Cochrane图书馆、Pubmed、EMbase、万方数据库、中国知网(CNKI)、中国生物医学文献数据库(CBM)或手工检索发表于2005年11月至2013年11月关于降压药联合他汀类药物对高血压患者BPV影响的随机对照试验(RCT),应用Rev Man5.0软件对数据进行Meta分析。结果共纳入6个RCT共830例患者,其中降压药联合他汀类药物(试验组)421例,单用降压药(对照组)409例。与对照组相比,试验组24 h平均收缩压(24hSBP)下降8.91 mmHg[95%CI(-13.96,-4.07),P<0.001],24h平均舒张压(24hDBP)下降4.39 mmHg[95%CI(-8.40,-0.39),P=0.030],24 h收缩压标准差(24hSSD)下降1.95 mmHg[95%CI(-2.32,-1.58),P<0.001],但两组24 h舒张压标准差(24hDSD)比较,差异无统计学意义(P=0.310)。结论降压药联合他汀类药物治疗高血压患者对24hSBP、24hDBP的改善疗效确切,同时能降低24hSSD,但对24hDSD的改善不明显。Objective To evaluate the effect of antihypertensive drug combined with statins on blood pressure variability（BPV） of hypertensive patients. Methods Cochrane Library,Pubmed,EMbase,Wanfang Database,CNKI,CBM were searched or a manual retrieval was done for randomized controlled trials（ RCTs） on antihypertensive drug combined with statins in the treatment of blood pressure variability of hypertensive patients from November,2005 to November,2013. The software RevMan 5. 0 was used to conduct a Meta-analysis on the selected studies. Results Six RCTs including 830 cases were analyzed,in which there were 421 cases treated with antihypertensive drug combined with statins（ experimental group） and 409 cases treated with antihypertensive drug alone（ control group）. Compared with control group,the mean 24-hour systolic blood pressure（24hSBP） decreased by 8. 91 mmHg[95% CI（- 13. 96,- 4. 07）,P〈0. 001],the mean 24-hour diastolic blood pressure（24hDBP） decreased by 4. 39 mmHg[95% CI（- 8. 40,-0. 39）,P = 0. 030],the 24-hour systolic standard deviation（24hSSD） decreased by 1. 95 mmHg[95% CI（- 2. 32,-1. 58）,P〈0. 001],but there was no significant difference in the 24-hour diastolic standard deviation（24hDSD） between two groups（ P = 0. 310）. Conclusion Antihypertensive drug combined with statins can reduce 24 hSBP,24hDBP and24 hSSD in hypertensive patients,but has no significant effect on the improvement of 24 hDSD.
