摘　　要：目的探讨左心房内径和炎性因子在心房颤动(AF)发生和发展的作用。方法 45例持续性AF患者和45例阵发性AF患者作为AF组,选取同期住院无心房颤动病史的窦性心律患者45例为对照组。研究对象行超声心动图检测左心房内径(LAD),行免疫比浊法和ELISA检测患者血清高敏C-反应蛋白(hs-CRP),白细胞介素-6(IL-6)和肿瘤坏死因子(TNF-α)。结果心房颤动组LAD值和hs-CRP、IL-6、TNF-α水平均高于对照组,差异有统计学意义(P<0.05)。AF患者LAD与hs-CRP、IL-6和TNF-α正相关(P<0.05)。与阵发性AF组比较,持续性AF组中、高危水平hs-CRP的LAD水平均明显增加,差异有统计学意义(P<0.05)。阵发性AF组和持续性AF组组内比较,高危水平hs-CRP患者的LAD水平均比中危组明显增加(P<0.05)。结论 AF组患者血清hs-CRP、IL-6和TNF-a水平与LAD有关。Objective To investigate the effect of subclinical inflammatory factors in the occurrence and development on atrial fibrillation（ AF）. Methods Forty-five patients with persistent AF and 45 patients with AF were selected as AF group,45 cases in the control group. The left atrial diameter（ LAD） was measured by ultrasonic cardiogram. Levels of serum high-sensitivity C-reactive protein（ hs-CRP）,interleukin-6（ IL-6） and tumor necrosis factor alpha（ TNF-α） were examined by turbidimetry and ELISA. Results LAD,serum hs-CRP,IL-6 and TNF- α levels were higher in AF group than those in the control group,the differences were statistically significant（ P 〈. 05）. There was a positive correlation between LAD and levels of inflammatory factors（ hs-CRP,IL-6 and TNF-α） in AF patients（ P 〈.05）. Compared with paroxysmal AF group,LAD levels in persistent AF group were significantly increased in high risk level hs-CRP,the difference was statistically significant（ P 〈. 05）. Comparison within group of paroxysmal and persistent AF group,LAD increased significantly in high level hs-CRP than in medium risk group（ P 〈. 05）.Conclusion There is a positive correlation relationship between serum hs-CRP,IL-6 and TNF-a levels and the size of LAD in AF patients.
