主动脉瘤附壁血栓、扩张形态和位置的CT血管成像表现及其相关性分析

摘　　要：目的 利用320排动态容积CT血管成像技术显示主动脉瘤附壁血栓、主动脉瘤位置和扩张形态，并对其相关性进行讨论。方法 回顾性分析2011年4月至2014年3月行320排动态容积CTA检查诊断为主动脉瘤的患者49例，总结分析其CTA表现特点及附壁血栓与主动脉瘤的位置及不同瘤体形态之间的相关性。结果 本组49例中，升主动脉瘤23例，降主动脉瘤2例，腹主动脉瘤24例；瘤体最大径1.02～9.60 cm，平均（5.68±1.49）cm；病变范围5.00～14.10 cm，平均（7.46±3.38）cm；主动脉瘤合并附壁血栓19例，主动脉瘤合并附壁血栓与腹主动脉瘤肾动脉下型[OR（95%CI）值17.484（2.280，134.093），P＝0.006]和瘤样扩张[OR（95%CI）值7.792（1.455，41.728），P＝0.016]相关。结论 腹主动脉瘤肾动脉下型和瘤样扩张与附壁血栓形成关系密切。Objective To study the correlation between the intraluminal thrombosis and the location as well the shape of the aortic aneurysm.Methods Forty-nine consecutive patients with aortic aneurysm detected by 320-slice dynamic volume CT from April, 2011 to March, 2014 in our hospital were recruited in this study. The CTA characteristics of the aortic aneurysm were recorded. The correlation between the intraluminal thrombosis and the location as well as the shape of the aortic aneurysm were analyzed.Results There were 49 patients with aortic aneurysm （23 in the ascending aorta, 2 in the descending aorta and 24 in the abdominal aorta）. The size and extent of the aneurysm were （5.68±1.49）cm, （7.46±3.38）cm, respectively. Nineteen patients had intraluminal thrombosis. There were statistically significant correlation between intraluminal thrombosis and the infrarenal type of abdominal aortic aneurysm[OR（95%CI）=17.484（2.280, 134.093）,P=0.006] as well as the aneurysmal dilatation [OR（95%CI）=7.792（1.455, 41.728）,P=0.016].Conclusion The study indicate significant correlation of the intraluminal thrombosis with infrarenal type of the abdominal aortic aneurysm and aneurysmal dilatation.
