摘 要:目的探讨HBV感染、饮酒及吸烟在原发性肝癌发病中的交互作用。方法以肝癌高发区广东顺德区为研究现场,选取306例原发性肝癌患者为病例组,306例正常者为对照组,运用相加模型分析HBV感染、饮酒及吸烟对原发性肝癌的交互作用。结果采用多因素Logistic回归模型调整年龄、性别等影响因素后,HBV感染与饮酒对原发性肝癌的发病有协同作用,协同效应指数为1.748;交互作用超额相对危险度为12.268;交互作用归因比为41.4%。HBV感染与吸烟对原发性肝癌的发病也有协同作用,协同效应指数为2.068;交互作用超额相对危险度为11.716;交互作用归因比为49.5%。结论 HBV感染与饮酒、HBV感染与吸烟之间存在相加交互作用,两者同时存在可增加患原发性肝癌的危险性。Objective The study aimed to explore the interaction of HBV infection, drinking, and smoking on the incidence of primary liver cancer (PLC). Methods Shunde district which most incidences of liver cancer were occurred in was selected as study site, 360 patients with PLC was selected as a case group, and 306 normal persons as a control group. Additive model was used to analyze the interaction of HBV infection, drinking, and smoking on the incidence of PLC. Results After adjusting for age, gender, and other influencing factors by multiple logistic regression model, there was an interaction of HBV infection and drinking on the incidence of PLC, with a synergistic effect index of 1.748. The relative excess risk of interaction was 12.268 and the attribution proportion of interaction was 41.4%. There was a synergistic effect of HBV infection and smoking on the incidence of PLC, with a synergistic effect index of 2.068. The relative excess risk of interaction was 11.716 and the attribution proportion of interaction was 49.5%. Conclusion There are additive interactions between HBV infection and drinking, as well as HBV infection and smoking. Both of them existed can increase the risk of the incidence of PLC.
