摘  要:目的观察持续声门下引流预防呼吸机相关性肺炎(VAP)的效果。方法对某院综合ICU科室收治的200例经口气管插管行机械通气(MV)患者病例随机分为试验组和对照组。试验组为持续声门下引流组,对照组为常规气道护理,未行持续声门下引流组。两组患者在年龄、性别、病情严重程度方面比较差异均无统计学意义。观察两组患者发生VAP的例数,并记录机械通气时间和住院时间,所得研究数据通过SPSS11.5软件包进行统计、分析。结果在两组患者机械通气时间和平均住院时间比较差异无统计学意义(P>0.05)的情况下,试验组104例,VAP发生率为4.80%,对照组96例,VAP发生率为21.90%,差异有统计学意义(P<0.05)。结论在机械通气时间和平均住院时间无差异的背景下,持续声门下引流可明显降低气囊上滞留物的下移,可以明显减少和延迟呼吸机相关性肺炎的发生。Objective The objective of the study was to observe the effectiveness of continuous subglottic drainage on ventilator-as- sociated pneumonia (VAP). Methods A total of 200 cases admitted to the general ICU department of our hospital after intubation and mechanical ventilation (MV) patients were randomly divided into a test and a control groups. The test group continued sound sects as the attract group, the control group of conventional airway care, did not undergo continuous subglottic drainage. No statisti- cal significances were observed in age, gender, and severity of incidence of both groups. The cases of VAP in both groups were ob- served, and the data of duration of mechanical ventilation and hospitalization time were statistically analyzed by using SPSS11.5 software. Results There were no statistical significances between duration of mechanical ventilation and average hospitalization time in both groups (P〉0.05). 104 cases of test group showed a VAP incidence of 4.8%. 96 cases of control group showed a VAP inci- dence of 21.9%, and the difference was statistically significant (P〈0.05). Conclusion Continuous subglottic drainage can signifi- cantly reduce and delay the occurrence of ventilator-associated pneumonia.
