摘 要:目的探讨系统性红斑狼疮(SLE)炎症状态与骨质疏松的相关性。方法将该院2008年10月至2013年10月诊治的SLE患者128例根据SLE疾病活动指数(SLEDAI)评价分为四组,即基本无活动为SLE 1组(34例),轻度活动为SLE 2组(38例),中度活动为SLE 3组(37例)及重度活动为SLE 4组(19例)。另选取同期该院健康体检者95例作为对照组,测定两组骨密度。结果 SLE患者骨量减少和骨质疏松总发生率[25.0%(32/128)]高于对照组[10.5%(10/95)],差异有统计学意义(P<0.01);SLE 1组患者骨量减少和骨质疏松发生率与对照组比较,差异无统计学意义(P>0.05);SLE2、3、4组患者骨量减少和骨质疏松总发生率明显大于SLE 1组,差异均有统计学意义(P<0.05);同时SLE 3、4组患者骨量减少和骨质疏松发生率显著高于SLE 2组,差异均有统计学意义(P<0.01)。进行logistic回归分析,发现SLEDAI积分、年龄、长期应用糖皮质激素是骨质疏松的危险因素(P<0.01)。结论 SLE炎症活动与骨质疏松的发生发展关系密切。Objective To investigate the association of the inflammatory state of systemic lupus erythematosus(SLE)and osteoporosis. Methods A total of 128 patients with SLE,who were treated in the hospital from October 2008 to October2013,were selected(SLE group) and divided into four subgroups according to the SLE disease activity index(SLEDAI) evaluation:SLE1 group with seldom activity(n=34),SLE2 group with light activity(n=38),SLE3 with moderate activity(n=37) and SLE4 group with serious activity(n=19). Simultaneously,other 95 healthy controls in the hospital were selected as the control group.The bone mineral densitywas deteced. Results The total occurrence rate of osteopenia and osteoporosis of SLE patients was[25.0%(32/128)],which was higher than that of the control group[10.5%(10/95)] with statistically significant difference(P〈0.01);the difference on occurrence rate of osteopenia and osteoporosis between SLE1 group and the control group had no statistical signifi cance(P〈0.05);the occurrence rate of osteopenia and osteoporosis in SLE2 group,SLE3 group and SLE4 group were obviously greater than that in SLE1 group with statistically significant difference(P〉0.05);the occurrence rate of osteopenia and osteoporosis of SLE3 group and SLE4 group was apparently higher than that of SLE2 group with statistically significant difference(P〈0.01).Logistic regression analysis showed that the SLEDAI score,age and long-term application of glucocorticoid were risk factors for osteoporosis(P〈0.01). Conclusion The inflammatory activity of SLE is closely related with the occurrence and development of osteoporosis.
