社区医院卫生学状况研究分析

摘　　要：目的掌握社区医院的卫生学状况,为社区医院指导医院内感染的控制重点和方向。方法对西安地区5家社区医院（观察组）2011-2013年的空气、物体表面、手卫生消毒、无菌物品、使用中的消毒液染菌量、紫外线强度进行卫生学监测,并与该地区5家二级医院（对照组）监测结果进行对比分析。结果观察组2011-2013年手卫生消毒合格率低于对照组,差异均有统计学意义（χ^2=7.371、6.061、11.849,P〈0.05）;观察组2011-2013年合计合格率低于对照组,差异均有统计学意义（χ^2=45.376、11.073、5.128,P〈0.05）;两组2011-2013年其他卫生学指标比较,差异均无统计学意义（P〉0.05）。观察组中使用中的消毒液染菌量、合计合格率逐年增长,年间比较,差异均有统计学意义（χ^2=14.617、54.402,P〈0.05）。结论社区医院的卫生学状况逐年在改善,但仍需重视及加强,尤其在手卫生消毒方面。Objective Master hygiene status of community health service station to guide the control keys and direction of the hospital-acquired infection. Methods It was monitored that air quality,object surface,hand hygiene and disinfection,sterile goods,the bacterium amount in disinfectant in use,ultraviolet intensity of 5 community health service stations（the observation group） in Xi′an from 2011 to 2013,whose results were compared and analyzed with another 5 grade Ⅱ hospital（the control group） in the region. Results The qualified rate of hand hygiene and disinfection in the observation group from 2011 to 2013 were lower than that of the control group,whose difference was statistically significant（χ^2=7.371、6.061、11.849,P〈0.05）. The total qualified rate in the observation group was lower than that of the control group. There was statistical significance in difference（χ^2=45.376、11.073、5.128,P〈0.05）.There were no differences in other health indications（P〈0.05）. It was increasing annually in the bacterium amount of disinfectant in use. The difference has statistical significance between the years（χ^2=14.617,P〈0.05）.Conclusion It has been improved in health status in community health service stations yearly. What we should especially focus on is hand hygiene and disinfection and constantly pay more attention to it.
