血浆脑钠肽前体在急性冠脉综合征患者冠脉病变风险及心功能评价中的价值

摘　　要：目的：探讨血浆脑钠肽前体（ NT-pro BNP）在急性冠脉综合征（ ACS）患者冠脉病变风险及心功能评价中的价值。方法测定商丘市第一人民医院心内科收治的100例ACS患者及50例同期健康体检者血浆NT-pro BNP、左室射血分数（ LVEF），并通过冠脉造影检查得出冠状动脉病变支数及Gensini评分。结果与对照组比较， ACS患者血浆NT-pro BNP水平、Gensini评分明显升高，而LVEF明显下降（ P﹤0.05）；且随着ACS临床严重程度及冠状动脉病变支数的增加，患者血浆NT-pro BNP水平、Gensini评分依次增加，LVEF依次下降（P﹤0.05）；血浆NT-pro BNP与冠脉病变支数、Gensini评分均呈线性正相关（r=0.711，0.835，P均﹤0.05），与LVEF呈线性负相关（ r=-0.415，P﹤0.05）。结论血浆NT-pro BNP与ACS患者冠状动脉病变及其严重程度、心功能有关，在ACS的诊疗和早期危险分层中具有重要意义。Objective To investigate the evaluation value of plasma NT-pro BNP concentration on coronary artery lesion and heart function in patients with acute coronary syndrome（ ACS）. Methods The plasma NT-pro BNP and left ventricular ejection fraction（ LVEF）100 ACS patients who were admitted in ICU and 50 healthy persons were detected, and the number of diseased coronary arteries and coronary angiography Gensini score were measured by syntax score assay. Results Compared with health controls,plasma NT-pro BNP levels and Gensini score were significantly higher,LVEF was significantly lower in ACS patients,and with the severity aggravating of ACS and number of diseased coronary arteries, BNP levels and Gensini scores were increased,LVEF was decreased（ P﹤0. 05 ）. There was significant positive relation between plasma NT-pro BNP and number of diseased coronary arteries and Gensini score（r=0. 711,0. 835,P﹥0. 05）, while negative relation was found with LVEF（r= -0. 415,P﹤0. 05）. Conclusions Plasma NT-pro BNP is correlated with coronary artery lesion and heart function in ACS patients. Plasma BNP level increased in patients with sepsis,and it plays a significant role in the early diagnosis and treatment and the risk stratification of ACS.
