摘　　要：目的观察Graves病患者治疗前后外周血CD28-T、CD4+CD28-T、CD8+CD28-T细胞亚群和可溶性CD28(sCD28)含量的表达变化,以分析其作为预后判断和病情监测生物学指标的临床意义。方法采用双单抗夹心ELISA法测定Graves病患者外周血中sCD28的含量;流式细胞术分析其外周血中CD3+T、CD4+T、CD8+T、CD28-T、CD4+CD28-T、CD8+CD28-T细胞亚群的表达百分率,并进行统计学分析。结果与健康人对照组相比,Graves病患者CD3+T、CD4+T细胞亚群表达百分率显著减少(P均<0.01),而CD28-T、CD4+CD28-T、CD8+CD28-T细胞亚群的表达百分率和sCD28含量均显著升高(P均<0.01);与治疗前比较,Graves病患者治疗后外周血CD28-T、CD4+CD28-T、CD8+CD28-T细胞亚群和sCD28浓度均显著下降,并呈恢复性改变(P均<0.01);且上述4种指标的表达水平与Graves病患者临床体征(甲状腺肿大程度、GO眶病)呈正相关关系(P均<0.01)。结论 Graves病患者外周血CD28-T、CD4+CD28-T、CD8+CD28-T细胞亚群和sCD28含量的表达水平与Graves病患者的病情密切有关,可能为Graves病的预后判断和病情监测提供依据。Objective To investigate the changes of CD28 - T, CD4＋ CD28 - T and CD8＋ CD28 - T cell subsets and soluble CD28 （ sCD28 ） levels in peripheral blood of patients with Graves＇ disease before and after the treatment, and analyze their significance in the monitoring and prognosis of the disease. Methods The level of sCD28 in peripheral blood of patients with Graves＇ disease was determined by the sandwich ELISA. The percentages of CD3＋ T, CD4 ＋ T, CD8 ＋ T, CD28 - T, CD4＋ CD28 - T and CD8 ＋ CD28 - T ceils were measured by flow cytometry. Then, the related statistical analysis was performed. Results Compared with the healthy control group, the percentages of CD3 ~ T and CD4 ~ T ceils in peripheral blood of patients with Graves＇ disease decreased significantly （P 〈 0.01 ）, while those of CD28 - T, CD4＋ CD28 - T and CD8＋ CD28 - T ceils and the level of sCD28 increased obviously （ P 〈 0.01 ）. The percentages of CD28 - T, CD4 ＋ CD28 - T and CD8 ＋ CD28 - T cells and the level of sCD28 in peripheral blood of patients with Graves＇ disease reduced significantly after treatment as eompared with before treatment （ P 〈 0.01 ）. Moreover, the percentages of CD28 - T, CD4＋ CD28 - T and CD8＋ CD28 - T cells and the level of sCD28 were positively related to the elinieal signs of Graves＇ disease such as the degree of goiter and Graves＇ ophthalmopathy （P 〈 0.01 ）. Conclusion The levels of CD28 - T, CD4 ＋ CD28 - T and CD8 ＋ CD28- T cells and sCD28 in peripheral blood of patients with Graves＇ disease are closely related to the condition of the disease, whieh may provide the evidence for the monitoring and prognosis of Graves＇ disease.
