摘 要:目的探讨Graves病患者外周血滤泡辅助性T细胞(follicular helper T cells,Tfh)和活化B细胞(CD19+CD83+B细胞)与疾病的关系。方法采用电化学发光法和流式细胞术分析新发Graves病患者和体检健康者外周血FT3、FT4、TSH、甲状腺自身抗体、Tfh和活化B细胞的水平,同时进行相关性分析。结果 Graves病患者Tfh为(3.05±1.33)%,高于健康人对照组的(1.15±0.59)%(P<0.05);Graves病患者活化B细胞为(7.77±3.42)%,高于对照组的(4.94±1.43)%(P<0.05);Graves病患者Tfh与TPO-Ab、Tg-Ab呈正相关(TPO-Ab:r=0.893,Tg-Ab:r=0.807,P均<0.01),而与TRAb无相关性(r=0.539,P>0.05);此外活化B细胞比例与其外周血中TPO-Ab呈正相关(r=0.895,P<0.01),而与Tg-Ab、TRAb无相关性(Tg-Ab:r=0.137,TRAb:r=0.398,P均>0.05)。Graves病患者外周血Tfh与活化B细胞呈正相关(r=0.702,P<0.05)。结论Graves病患者外周血中Tfh及活化B细胞可能参与Graves病的发生和发展。Objective To investigate the correlation of the levels of circulating follicular helper T cells (Tfh) and activated B cells in peripheral blood from Graves' disease patients with the development of Graves' disease. Methods The levels of FT3, FT4, TSH, thy- roid autoantibodies such as TPO-Ab, Tg-Ab and TRAb, Tfh and activated B cells in peripheral blood samples from onset patients with Graves' disease and healthy controls were determined by electrochemiluminescence immunoassay (ECLIA) or flow cytometry (FCM), and the correlations between them were analyzed. Results The percentages of Tfh [ ( 3.05 ± 1.33 ) % ] and activated B cells [ (7.77 ± 3.42) % ] in Graves' disease patients were significantly higher than those in the healthy controls [ ( 1.15± 0.59) % ] and [ (4.94± 1.43) % ] (P 〈 0. 05). The percentage of Tfh cells was positively related to the levels of TPO-Ab ( r = 0. 893, P 〈 0.01 ) and Tg-Ab (r = 0. 807, P 〈 0.01 ) in the patients with Graves' disease, but unrelated to the level of TRAb ( r = 0. 539, P 〉 0.05 ). The percentage of activated B cells was positively related to the level of TPO-Ab (r = 0. 895, P 〈 0.01 ) in the patients with Graves' disease, but unrelated to the levels of Tg-Ab ( r = 0. 137, P 〉 0.05 ) and TRAb ( r = 0. 398, P 〉 0.05). Moreover, the percentage of Tfh was positively related to that of activated B cells in Graves' disease patients ( r = 0. 702, P 〈 0. 05). Conclusion Tfh cell and ac- tivated B cell may be involved in the onset and development of Graves' disease.
