过敏性紫癜患儿血清hsCRP、CysC和尿mALB水平及其相关性

摘　　要：目的探讨过敏性紫癜(HSP)患儿血清超敏C反应蛋白(hsCRP)、胱抑素C(Cys C)和尿微量白蛋白(mALB)水平及其相关性。方法检测HSP患儿72例(HSP组)和健康儿童25例(对照组)血清hsCRP、Cys C和尿mALB水平,分析其相关性。结果 HSP组血清hsCRP、Cys C和尿mALB分别为(21.18±7.99)mg/L、(1.61±0.54)mg/L和(30.53±12.84)mg/L,均高于对照组的(2.84±1.07)mg/L、(0.67±0.48)mg/L和(5.96±2.09)mg/L(P<0.01)。血清hsCRP与血清Cys C、尿mALB呈显著正相关(r=0.465、r=0.708,P<0.01);血清Cys C与尿mALB也呈显著正相关(r=0.594,P<0.01)。32例肾损害HSP患儿血清hsCRP、Cys C和尿mALB水平分别为(25.92±7.17)mg/L、(1.86±0.46)mg/L和(41.88±9.38)mg/L,均高于40例无肾损害HSP患儿的(17.38±6.50)mg/L、(1.41±0.52)mg/L和(21.45±6.32)mg/L(P<0.01)。结论 HSP患儿血清hsCRP、Cys C和尿mALB水平的升高与炎性反应程度密切相关,可作为判断HSP患儿肾损害的重要指标。Objective To explore serum high-sensitivity C reactive protein （hsCRP） and Cystatin C（Cys C） and urine microalbumin（mALB） in children with Henoch-Seh0nlein purpura（HSP） and their correlation. Methods Serum hsCRP and Cys C and urine mALB were detected in 72 children with HSP（HSP group）and 25 healthy children（control group）. Correlation among three of them was analyzed. Results Serum hsCRP and Cys C and urine mALB in HSP group were （21.18±7.99） mg/ L, （1.61 ± 0. 54） mg/L and （30. 53 ±12.84） mg/L, which were significantly higher than （2.84 ± 1.07） mg/L, （0.67±0.48） mg/L and （5.96±2.09） mg/L in control group（P〈0. 01）. Serum hsCRP was positively correlated with serum Cys C and urine mALB （r=0. 465, r=0. 708, P〈0. 01）. Serum Cys C was positively correlated to urine mALB（r=0. 594, P%0. 01）. Serum hsCRP and Cys C and urine mALB in 32 HSP children with renal damage were （25.92±7.17） mg/L, （1.86±0.46） mg/L and （41.88±9.38） mg/L,which were significantly higher than （17.38±6.50） mg/L, （1.41±0.52） mg/L and （21.45±6.32） mg/L in 40 HSP children without renal damage（P〈0. 01）. Conclusion High serum hsCRP and Cys C and urine rnALB are closely associated with the degree of inflammation reaction,which can be taken as important indicators for diagnosing renal damage in children with HSP.
