摘  要:目的探讨糖尿病(DM)椎体束、周围感觉、运动及自主神经功能。方法应用经皮层、脊髓电刺激运动诱发电位(MEP)、神经传导速度(NCV)及交感神经皮肤反应(SSR)技术对50例DM进行观察。结果MEP异常率82%(41/50),表现皮层、脊髓刺激MEP潜伏期延迟、波形分化欠佳,部分伴有中枢运动传导时间(CMCT)异常;NCV异常率70%,主要表现运动神经传导速度(MCV)延迟占65%(260/400),感觉神经传导速度(SCV)延迟占75%(150/200);SSR异常率76%(38/50),主要表现潜伏期延迟及波幅减低。MEP、NCV、SSR异常率与病程长短呈正相关;空腹血糖水平对MEP、NCV、SSR结果无明显影响。结论 DM患者的锥体束及周围感觉运动神经、自主神经功能均存在明显异常。MEP结合NCV、SSR可对DM的中枢及周围神经系统功能提供全面客观指标。【Objective】To study the function of pyramidal tract, Autonomic nerve and peripheral nerve in patients with diabetes mellitus(DM).【Methods】Motor evoked potentials(MEP) by electro-stimulation. Of cerebral cortex and spinal cord, neuropathy sympathetic skin response(SSR) and nerve conduction velocity(NCV) were performed in 50 patients with DM.【Results】The abnormal rate of MEP was 82%(41/50). The main changes were the prolonged latency and unclear wave, form Central motor conduction time(CMCT) was prolonged in six patients. The abnormal rate of SSR was 76%(38/50), the main changes were prolongation of latency and remarkable decreases in amplitude. The abnormal rate of NCV was 70%, the main changes were prolonged motor conduction velocity(MCV) and sensory conduction velocity(SCV). There was positive correlation between the course of disease and the abnormal rate of MEP,NCV and SSR. There was no significant relation between the result of MEP, NCV, SSR and the level of fasting blood sugar. 【Conclusions】Abnormalities of pyramidal tract, autonomic nerve and peripheral nerve may occurin patients with DM, MEP, SSR and NCV examinations may provideobjective indexes for function of nerve system in patients with DM.
