摘  要:目的探讨双源CTFlash模式在肺动脉成像中对比剂应用的优化方案.资料与方法收集临床疑诊为肺动脉栓塞的90例患者,行双源CTFlash模式肺动脉成像,采用人工智能触发扫描,选定肺动脉主干为感兴趣区,随机分为A、B、C组,每组各30例,分别给予35、40、45ml对比剂,注射速度5.0ml/s,然后以相同速度注射生理盐水30ml,测量3组肺动脉干、S1段肺动脉、S10段肺动脉、上腔静脉的CT值,并对3组肺动脉图像质量及上腔静脉硬化伪影进行评价.结果A组各段肺动脉增强CT值>250HU者占73.33%~90.00%,低于B、C组的93.33%~100.00%(P<0.05).A组平均肺动脉图像质量评分为(4.37±0.63)分,低于B、C组的(4.91±0.68)分和(4.88±0.54)分(P<0.05),B、C组间评分差异无统计学意义(P>0.05).上腔静脉硬化伪影分析:C组伪影明显多于A、B组(P<0.05).结论应用双源CTFlash模式肺动脉成像时,使用较低剂量对比剂(40ml对比剂+30ml生理盐水)及较高的注射速度(5.0ml/s)可以得到满足诊断要求的图像.Purpose To explore the optimum contrast medium injection protocol of dual-source CT Flash scan in pulmonary angiography. Materials and Methods Ninety cases suspected pulmonary embolism underwent CT pulmonary angiography using dual- source CT Flash scan. The region of interest of triggering position was set in the lumen of the main pulmonary artery (MPA). All the subjects were divided into group A, B and C, with each group of 30 cases. Patients in group A, B and C were injected 35 ml, 40 ml and 45 ml contrast medium respectively with an injection flow rate of 5.0 ml/s, followed by a saline chaser of 30 ml. CT value of pulmonary arteries (MPA, S1 artery and S10 artery) and superior vena cava were measured. Image quality of pulmonary arteries and artifact of superior vena cava were also analyzed. Results Percentage of optimal attenuation in pulmonary arteries (〉250 HU) was lower in group A than that in group B and C (73.33%-90.00% vs. 93.33%-100.00%, P〈0.05). The image quality score in group A (4.37±0.63) was lower than that of group B (4.91 ±0.68) and group C (4.88±0.54) (P〈0.05). There was no significant difference between group B and group C (P〉0.05). The artifact of superior vena in group C was much more than that in other groups (P〈0.05). Conclusion Pulmonary angiography using dual-source CT Flash scan with low dose contrast medium and relatively high flow rate can meet the requirement of the diagnosis.
