摘 要:目的探讨彩色多普勒超声(color Doppler ultrasound,CDU)对乳腺癌新辅助化疗(neoadjuvant chemotherapy,NAC)疗效的评估价值。方法 52例乳腺癌患者分别于NAC前、后行CDU检查,比较NAC前、后病灶大小、血流分级、血流阻力指数等指标,评估化疗疗效。结果 CDU提示完全缓解8例,部分缓解32例,维持11例,进展1例,总有效率为76.92%;NAC前病灶长、宽、厚、面积、体积分别为(3.42±0.42)cm、(2.70±0.63)cm、(1.87±0.40)cm、(9.01±2.13)cm2、(16.23±4.21)cm3,治疗后分别为(2.30±0.38)cm、(2.14±0.53)cm、(1.53±0.40)cm、(5.00±1.83)cm2、(7.88±3.27)cm3,差异有统计学意义(P〈0.05);NAC前边界清晰38例、有周边声带21例、内部回声强11例、后方回声异常14例、形态不规则8例、血流分级Ⅱ-Ⅲ级49例,NAC后分别为23、10、34、40、32、30例,差异均有统计学意义(P〈0.05);有效组NAC后血流阻力指数(0.62±0.14)明显低于无效组(0.77±0.13)与本组NAC治疗前(0.77±0.23)(P〈0.05)。结论 CDU可发现乳腺癌NAC化疗前、后相关参数的明显变化,对评估NAC疗效有重要价值。Objective To explore the value of color Doppler ultrasound(CDU)to evaluating the effect of neoadjuvant chemotherapy(NAC)on breast cancer.Methods Fifty-two patients with breast cancer received CDU before and after NAC,and were compared the tumor size,blood flow grading,resistance index and other related indicators to assess the therapeutic effect.Results CDU revealed complete remission in 8patients,partial remission in 32,no change in 11,and progressive disease in 1,with the total effective rate of 76.92%.The length,width,thickness,area and volume of the cancer lesions were(3.42±0.42)cm,(2.70±0.63)cm,(1.87±0.40)cm,(9.01±2.13)cm2 and(16.23±4.21)cm3respectively before NAC,showing significant differences in comparison with those after NAC((2.30±0.38)cm,(2.14±0.53)cm,(1.53±0.40)cm,(5.00±1.83)cm2,(7.88±3.27)cm3)(P〈0.05).CDU showed clear boundaries,surrounding vocal cords,strong internal echo,abnormal echo back,irregular shape,and blood flow gradeⅡorⅢ was discovered in 38,21,11,14,8and 49 patients before NAC,and in 23,10,34,40,32 and 30patients after NAC,showing significant differences(P〈0.05).The blood flow resistance index was significantly lower in effective group(0.62±0.14)than that in void group after NAC(0.77±0.13)and in effective group before NAC(0.77±0.23)(P〈0.05).Conclusion CDU can show the significant changes in related indicators of breast cancer lesion after NAC,with an important value to assessing the therapeutic effect of NAC.
