血清PA、ALB和CRP在小儿脓毒症急性期和恢复期的水平变化

摘　　要：目的探讨血清前白蛋白(PA)、白蛋白(ALB)和C反应蛋白(CRP)在小儿脓毒症急性期和恢复期的水平变化。方法采用免疫比浊法和溴甲酚绿法检测51例脓毒症患儿急性期(A组)和恢复期(B组)及30例正常体检儿童(C组)血清PA、ALB和CRP水平。结果 A组PA水平低于B组和C组[(87.56±38.40)mg/L vs.(275.03±49.19)mg/L和(269.63±42.37)mg/L](P<0.01)。A组ALB水平低于B组和C组[(39.32±7.61)g/L vs.(48.17±4.58)g/L和(48.19±4.30)g/L](P<0.05)。A组CRP水平高于B组和C组[(72.17±60.05)mg/L vs.(7.40±5.40)mg/L和(6.04±2.79)mg/L](P<0.05)。结论急性期脓毒症患儿血清PA和ALB降低,而CRP升高,三者均可作为诊断早期脓毒症的敏感性炎症指标。Objective To explore the changes of serum prealbumin（PA） ,albumin （ALB） and C reactive protein（CRP） in acute and recovery stages of pediatric sepsis. Methods Serum PA, ALB and CRP were detected in 51 children with pediatric sepsis in acute stage（group A） and in recovery stage （group B） and 30 healthy children（group C） by immunoturbidimetry and bromocresol green methods. Results Serum level of PA was lower in group A than that in groups of B and C[（87.56±38. 40） mg/L vs. （275.03±49.19） mg/L and （269.63±42.37） mg/L]（P〈0. 01）. So did the levels of ALB [（39.32±7.61） g/L vs. （48.17±4. 58） g/L and （48. 19±4. 30） g/L]（P〈0. 05）. But serum level of CRP was higher in group A than that in groups of B and C[（72.17±60.05） mg/L vs. （7.40±5.40） mg/L and （6.04±2. 79） mg/L]（P〈0. 05）. Conclusion Serum levels of PA and ALB are reduced, but serum CRP is elevated in the children with pediatric sepsis in acute stage, which can be taken as the sensitive indicators of inflammation in diagnosing pediatric septsis.
