摘 要:目的:观察胃转流术(gastric bypass,GBP)对2型糖尿病大鼠的降糖效果及对血清二肽基肽酶-4(dipeptidyl peptidase-Ⅳ,DDP-Ⅳ)和胰高血糖素样-1(glucagon-like peptide-1,GLP-1)的影响.方法:健康♂SD大鼠随机分为正常手术组(NO组)、正常对照组(NC组)、糖尿病手术组(DO组)、糖尿病对照组(DC组).术前及术后第1、2、4、8周分别测各组空腹血糖(fasting plasma glucose,FPG)和血清DDP-Ⅳ、GLP-1.结果:GBP术后8 wk,DO组FPG由术前的20.21mmol/L±2.14 mmol/L下降至8.50 mmol/L±2.19 mmol/L(P<0.05).DO组术后1 wk血清DPP-Ⅳ开始下降,至术后第8周由术前的387.35 ng/L±46.75 ng/L降至141.57 ng/L±19.32 ng/L(P<0.05).DO组、NO组GBP术后第8周空腹GLP-1分别由术前的7.38 pmol/L±1.71 pmol/L和7.23 pmol/L±1.59 pmol/L升高到17.80 pmol/L±1.39 pmol/L和15.48 pmol/L±1.21 pmol/L(P<0.05),术后各时相点DO组的GLP-1明显高于相应时间点DC组和NC组(P<0.05).结论:GBP能明显降低2型糖尿病大鼠的血糖水平;DDP-Ⅳ和GLP-1的变化可能为其降糖机制之一.AIM: To observe the hypoglycemic effect of gas- tric bypass operation and its influence on dipep- tidyl peptidase-IV (DDP-IV) and glucagon-like peptide-1 (GLP-1) in type 2 diabetes rats. METHODS: Healthy male SD rats were random- ly divided into a type 2 diabetes operation group (DO group), a type 2 diabetes control group (DC group), a normal operation group (NO group) and a normal control group (NC group). Fast- ing plasma glucose concentration, DDP-IV and GLP-1 were measured preoperatively and 1, 2, 4 and 8 wk postoperativeIy.RESULTS: Fasting blood glucose in the DO group decreased from 20.21 mmol/L ±2.14 mmol/L preoperatively to 8.50 mmol/L ±2.19 mmol/L (P 〈 0.05) at week 8 after gastric bypass operation. Compared with the preoperative val- ue, the fasting blood DDP-IV in the DO group begun to decline from the first week postopera- tively, and decreased from 387.35 ng/L ±46.75 ng/L preoperatively to 141.57 ng/L ± 19.32 ng/ L (P 〈 0.05) at week 8 after gastric bypass opera- tion. GLP-1 in the DO and NO groups GLP-1 in- creased from 7.38 pmol/L± 1.71 pmol/L and 7.23 pmol/L± 1.59 pmol/L preoperatively to 17.80 pmol/L ± 1.39 pmol/L and 15.48 pmol/L ± 1.21 pmol/L (P 〈 0.05) at week 8 after gastric bypass operation. GLP-1 level was significantly higher in the DO group than in the DC and NC groups at corresponding time points (P 〈 0.05). CONCLUSION: Gastric bypass operation can markedly reduce plasma glucose in type 2 dia- betes rats possibly via altering serum DPP-Ⅳand GLP-1.
