摘　　要：目的:探讨采用Epley管石复位法联合Semont管石解脱法治疗后半规管良性阵发性位置性眩晕(BPPV)的疗效。方法:将150例确诊为单侧后半规管BPPV的患者随机分为A、B、C三组,A组(53例)给予Epley管石复位法治疗,B组(44例)给予Semont管石解脱法治疗,C组(53例)给予Epley管石复位法联合Semont管石解脱法治疗,分别记录患者治疗次数,统计治疗后疗效,并随访至治愈后3个月。结果:A组1次治愈38例(72%),2次治愈43例(81%),3次治愈45例(85%);B组分别为30例(68%)、35例(80%)、37例(84%);C组分别为47例(89%)、50例(94%)、52(98%)。C组治愈率明显高于A、B两组(3组1次治愈率相比χ2=6.777,P<0.05,3次治愈率相比χ2=6.647,P<0.05)。随访至治愈后3个月,A组复发6例,B组复发5例,C组复发1例。结论:Epley管石复位法联合Semont管石解脱法治疗后半规管BPPV,1次治愈率高,患者所需就诊次数减少,治疗后复发率低,适合临床应用。Objective：To investigate the treatment of benign positional paroxysmal vertigo of posterior semicircular canal by Epley maneuver combined with Semont maneuver.Method：One hundred and fifty patients with benign positional paroxysmal vertigo of posterior semicircular canal were randomly divided into three groups：group A,B and C.Patients in group A were treated by Epley maneuver and patients in group B were treated by Semont maneuver.Patients in group C were received the treatment of Epley maneuver combined with Semont maneuver.We recorded the times of treatments in different groups respectively.Statistics of treatment effects and follow-up studies with 3months after the recovery were assessed.Result：The cure rate of the canalith repositioning on the primary,secondary and tertiary treatment in group A was respective 72%（38/53）and 81%（43/53）and85%（45/53）,in group B was 68%（30/44）and 80%（35/44）and 84%（37/44）,in group C was 89%（47/53）and94%（50/53）and 98%（52/53）.The cure rate in group C is significantly higher than group A and group B（χ^2=6.777,P〈0.05;χ^2=6.647,P〈0.05）.3months after recovery 6patients in group A,5patients in group B and 1patient in group C were relapsed.Conclusion：By the use of Epley maneuver combined with Semont maneuver in the treatment of benign positional paroxysmal vertigo of posterior semicircular canal,the primary cure rate was increased and the numbers of treatments were reduced and the relapse was decreased.It is suitable to use Epley maneuver combined with Semont maneuver in the clinic.
