老年慢性阻塞性肺疾病患者记忆功能障碍的干预效果

摘　　要：目的探讨慢性阻塞性肺疾病(COPD)患者早期记忆功能障碍干预的新方法,以改善患者的生活质量。方法对存在记忆障碍的138例COPD患者,按照年龄、性别、受教育年限、肺功能进行随机分层抽样,选取70例COPD患者分为干预组和对照组,每组35例。干预组在常规治疗基础上建立健康生活方式,同时进行记忆训练、肺功能训练和氧气吸入,对照组接受常规治疗,6月后观察干预疗效。结果 6月后,干预组的行为记忆测验第2版(RBMT-Ⅱ)标准评分及回忆被藏物品、图片再认、立即回忆故事、故事延迟回忆、脸部再认、路线延迟回忆、立即回忆信件、信件延迟回忆和日期9个维度的初评分均高于对照组,差异有统计学意义(P<0.05或P<0.01)。结论护理干预可以降低或延缓COPD患者记忆障碍的发展。Objective To explore the prevalence of memory disorders in elderly patients with chronic obstructive pulmonary disease（ COPD）,and to provide basis advices for the improvement of the life quality of the patients. Methods138 patients with COPD complicated with memory disorder were selected according to a multi-stage stratified cluster sampling method. After cluster sampling,70 participants were randomly assigned into intervention group（ n = 35） or control group（ n = 35）.Participants in the intervention group received a healthy mode of life plus memory training,pulmonary function exercise and oxygen uptake on the base of usual care for six months.Participants in the control group received routine treatment. Results After six months,the standard scores of rivermead behavioral memory test-second edition（ RBMT-Ⅱ） and the nine factor scores such as hidden goods recall,picture recognition,immediately story recall,story delayed recall,faces recall,letter delayed recall,route delayed recall,immediately letters recall and date were significantly higher in the intervention group than those in the control group（ P〈0. 05 and P〈0. 01）. Conclusions Nursing intervention can reduce or delay the development of the memory disorders among elderly patients with COPD.
