摘  要:目的了解医院屎肠球菌和粪肠球菌的耐药性,为临床治疗和防止医院感染提供依据。方法回顾性分析2012年1-12月临床分离的184株屎肠球菌和147株粪肠球菌对抗菌药物的耐药性,菌株经WalkAway 96PLUS自动化微生物鉴定仪PC20复合板进行鉴定和药敏试验,数据采用WHONET5.6软件进行统计处理。结果 2012年1-12月临床共分离5 813株病原菌,其中分离出屎肠球菌184株占3.2%,粪肠球菌147株占2.5%;尿液中粪肠球菌检出75株占51.0%,屎肠球菌检出101株占54.9%;屎肠球菌对青霉素G、氨苄西林、环丙沙星、红霉素、左氧氟沙星、四环素、庆大霉素、利福平、链霉素、氯霉素、喹奴普汀/达福普汀、呋喃妥因、万古霉素和利奈唑胺的耐药率依次为95.1%、94%、94%、93.5%、88.6%、80.4%、72.8%、69.6%、65.8%、40.8%、38%、35.9%、1.6%和0.5%;粪肠球菌对喹奴普汀/达福普汀、红霉素、四环素、链霉素、利福平、庆大霉素、氯霉素、环丙沙星、左氧氟沙星、青霉素G、呋喃妥因、氨苄西林、万古霉素和利奈唑胺的耐药率依次为90.2%、87.1%、78.2%、55.8%、53.1%、52.4%、50.3%、46.9%、39.5%、17%、16.3%、15%、1.4%和0.7%。结论屎肠球菌和粪肠球菌对临床大部分抗菌药物的耐药性相差很大,主要引起泌尿道感染,临床必须根据药敏结果选用抗菌药物。OBJECTIVE To investigate drug resistance of Enterococcus f aecium and Enterococcus f aecalis in our hospital ,so as to guide rational clinical application of antibacterials and prevent nosocomial infections .METHODS Drug resistance of E . f aecium and E . f aecalis ,which were collected during Jan .2012 to Dec .2012 ,were retrospectively analyzed .The strains were identified by automated microbial identification system PC 20 composite panels of WalkAway 96 PLUS .The susceptibility test was also done by this device .The data were statistically analyzed by WHONET5 .6 software .RESULTS A total of 5 813 strains of pathogenic bacteria were collected during Jan .2012 to Dec .2012 including E . faecium accounting for 3 .2% (184 strains) and E . faecalis accounting for 2 .5% (147 strains) .A total of 75 strains of E . f aecium accounting for 50 .1% and 101 strains of E . f aecalis accounting for 54 .9% were derived from the urine .Resistance rates of E . f aecium to penicillin G , ampicillin ,ciprofloxacin ,erythromycin ,levofloxacin ,tetracycline ,high concentrations of gentamicin ,rifampicin , high concentrations of streptomycin ,chloramphenicol ,quinupristin/dalfopristin ,nitrofurantoin ,vancomycin and linezolid were 95 .1% ,94% ,94% ,93 .5% ,88 .6% ,80 .4% ,72 .8% ,69 .6% ,65 .8% ,40 .8% ,38% ,35 .9% , 1 .6% and 0 .5% .Resistance rates of E . f aecalis to quinupristin/dalfopristin ,erythromycin ,tetracycline ,high concentrations of streptomycin ,rifampicin ,high concentrations of gentamicin ,chloramphenicol ,ciprofloxacin , levoflocacin ,penicillin G ,nitrofurantoin ,ampicillin ,vancomycin and linezolid were 90 .2% ,87 .1% ,78 .2% , 55 .8% ,53 .1% ,52 .4% ,50 .3% ,46 .9% ,39 .5% ,17% ,16 .3% ,15% ,1 .4% and 0 .7% .CONCLUSIONS There was big difference in resistance of E . f aecium and E . f aecalis to most clinical antibacterials .E . f aecium and E . f aecalis mainly caused urinary tract infections .Antibiotics should be chosen according to drug sensitivity test in the clinical practice because of the signifi
