摘  要:目的探讨胆碱能抗炎通路被烟碱激活后对急性肺损伤的治疗作用,并研究其作用机制,为急性肺损伤的治疗寻找新的途径。方法将用油酸制备的32只急性肺损伤/急性呼吸窘迫综合征(ALI/ARDS)雄性大鼠模型随机分为4组,每组8只,4组分别为对照组、烟碱处理组(烟碱组)、ARDS组、烟碱与α-银环蛇毒素处理组(蛇毒素组),试验中测肺湿干重比、氧分压,制备肺组织病理切片,采用WESTERN-BLOTTING法检测肺组织中NF-κB p65、HMGB1(高迁移率族蛋白)值,ELISA法测大鼠血液中白介素-6(IL-6)值。结果 ARDS组、蛇毒素组大鼠肺组织病理改变较烟碱组明显加重;IL-6的浓度、动脉血氧分压(PaO2)、NF-κB p65及HMGB1的灰度值、肺湿干重比(W/D),ARDS组及蛇毒素组与烟碱组相比,差异有统计学意义(P<0.05)。结论烟碱通过作用于烟碱受体的α7亚基激动胆碱能抗炎通路,能明显减少炎症因子的表达,对肺组织具有保护作用。OBJECTIVE To discuss the therapeutic effect of cholinergic anti-inflammatory pathway activated by nicotine to acute lung injury ,and study the mechanism of action ,so as to find new way to treat acute lung injury . METHODS Thirty two male rats with acute lung injury /acute respiratory distress syndrome (ALI/ARDS ) prepared by oleic acid were randomly divided into four groups and each group had 4 rats .The four groups were control group ,nicotine treatment group(nicotine group) ,ARDS group ,nicotine andα-coral snake toxin treatment group (snake toxin group) .The wet and dry weight of lung(W/D) ,oxygen partial pressure ,preparation of lung tissue pathological section were all checked in the experiment .And the values of NF-κB p65 ,HMGB1(High Mobility Group Box 1) were tested by WESTERN-BLOTTING methods .The value of interleukin-6(IL-6) in the blood of mice was tested by ELISE method .RESULTS The pathology changes in nicotine group were gently than that in ARDS group and snake toxin group .The concentration of IL-6 ,PO2 ,the gray value of NF-κB p65 and HMGB1 ,W/D were significantly among different ARDS group , snake toxin group and nicotine group ( P〈0 .05) .CONCLUSION By acting on the nicotinic receptor α7 subunit , nicotine can activate the cholinergic antiinflammatory pathway and can significantly reduce the expression of inflammatory factors ,which will play a protective role in lung tissue .
