热休克蛋白70在胎膜早破孕妇羊膜腔感染中的早期诊断意义

摘　　要：目的探讨在胎膜早破的孕妇的热休克蛋白70(HSP 70)对合并羊膜腔感染的早期诊断意义。方法选择2011年9月-2013年3月住院分娩的胎膜早破孕妇40例,将20例足月和20例足月前胎膜早破孕妇,根据宫壁细胞细菌培养和涂片检查,阳性者进入足月胎膜早破合并羊膜腔感染高风险组(试验1组)或足月前胎膜早破合并羊膜腔感染高风险组(试验2组);阴性者进入足月胎膜早破无感染组(对照1组)或足月前胎膜早破无感染组(对照2组),提取孕妇血清和外周血单个核细胞(PBMCs),用酶联免疫吸附测定(ELISA)技术检测血清和PBMCs中HSP 70含量。结果试验1和2组的PBMCs胞内HSP 70的表达明显增加,其平均值分别为(595.3±34.1)pg/ml和(583.6±52.8)pg/ml;而对照1和2组的PBMCs胞内HSP 70分别为(203.4±39.3)pg/ml和(191.5±28.8)pg/ml,试验1组与对照1组及试验2组与对照2组相比,PBMCs胞内HSP水平差异有统计学意义(P<0.05);血清中HSP 70在胎膜早破合并羊膜腔感染孕妇体内有增加,但其水平无PBMCs胞内HSP 70的表达高,试验1和2组的血清HSP 70分别为(301.4±60.1)pg/ml和(310.7±33.2)pg/ml,与对照1和2组相比,差异均有统计学意义(P<0.05)。结论 HSP70可作为早期、快速、准确预测胎膜早破孕妇合并羊膜腔感染的指标之一。OBJECTIVE To discuss the significant of heat shock protein 70 （HSP 70 ） in early diagnosis of premature rupture of membrane complicated with amniotic infections .METHODS Totally 40 pregnant women with premature rupture of membranes ,who were hospitalized to give birth from Sep 2011 to Mar 2013 ,were enrolled in the study ;according to the results of bacterial culture of uterine wall cells and the smear examination ,the participants with positive results were assigned as the full term premature rupture of membrane complicated with high risk of amniotic infections group （ the experimental group 1） or the premature rupture of membranes before full term complicated with high risk of amniotic infections group （ the experimental group 2）;the participants with negative results were assigned as the full term premature rapture of membrane with infections （ the control group 1） or the premature rupture of membrane before full term without infections group （ the control group 2）;the serum and peripheral blood mononuclear cells （PBMCs） were extracted from the pregnant women ,and the contents of HSP 70 in the serum and PBMCs were determined by using enzyme-linked immunosorbent assay （ELISA） .RESULTS The expression of HSP 70 in the PBMCs was significantly increased in the experimental group 1 and the experimental group 2 ,with the average value of （595 .3 ± 34 .1）pg/ml and（583 .6 ± 52 .8）pg/ml , respectively ;while the content of HSP 70 in the PBMCs was （203 .4 ± 39 .3）pg/ml in the control group A ,（191 .5 ± 28 .8）pg/ml in the control group 2 ,and there was significant difference in the content of HSP in the PBMCs between the experimental group 1 and the control group A or between the experimental group 2 and the control group 2 （P〈0 .05） .The level of HSP 70 in the serum was elevated in the premature rupture membrane pregnant women complicated with amniotic infections but was not so high as the level of HSP 70 in the PBMCs ;the level of serum HSP70 was （301 .4 ± 60 .1�
