摘　　要：目的：探讨孕产期保健服务对低危孕妇分娩结局的影响。方法：将2011年11月~2012年11月在北方学院附属第一医院产科门诊就诊的低危孕妇随机分为试验组和对照组,试验组479例,由高年资助产士定期产检,在孕期各阶段实施多项保健服务;对照组485例,接受常规产科门诊检查;比较两组的分娩结局。结果：两组侧切自然分娩率、自然产率、剖宫产率及新生儿体重≥3 800 g的比例,阴道分娩孕妇总产程时间,产后出血量,因头盆不称、羊水过少、胎膜早破、巨大儿、枕后位、脐带绕颈及其他原因的剖宫产率比较差异有统计学意义（P〈0.05）。结论：积极开展孕产期保健服务项目可有效降低剖宫产率、提高阴道产率、缩短产程时间、减少产后出血量,对母儿日后生活质量有重要意义。Objective： To explore the effect of maternal health care service on delivery outcome of low - risk pregnant women. Methods： The low - risk pregnant women from obstetric outpatient of the hospital from November 2011 to November 2012 were randomly divided into two groups： 479 pregnant women in experimental group received regular prenatal examination and multiple health care services during different trimesters of pregnancy provided by the experienced midwives ; 485 pregnant women in control group received routine obstetric outpatient examination; the delivery outcomes in the two groups were compared. Results： There were statistically significant differences in spontaneous labor rate of episiotomy, spontaneous labor rate, cesarean section rate, the proportion of neonates with body weight≥ 3 800 g, the total time of vaginal delivery, the volume of postpartum hemorrhage, cesarean section rates caused by cephalopelvic disproportion, oligo- amnios, premature rupture of fetal membrane, macrosomia, occipitoposterior, cord around neck and other reasons between the two groups （ P 〈 0.05 ） . Conclusion ： Actively promoting maternal health care service project can effectively reduce cesarean section rate, improve vaginal delivery rate, shorten the stages of labor and reduce the volume of postpartum hemorrhage, which has important significance for impro- ving quality of life of mothers and their babies.
