地震灾害后再生育妇女妊娠流产状况的调查研究

摘　　要：目的 探讨地震灾害对再生育妇女月经、妊娠及妊娠结局的影响.方法 选取2011年1月～2012年12月妇产科门诊及病房的296例有再生育意愿的妇女为研究对象,按年龄将其分为＜35岁组（77例）、35～39岁组（163例）和≥40岁组（56例）.对其月经、妊娠、妊娠结局及其他影响因素进行统计学分析.结果 296例再生育妇女中,震前、震后月经周期、月经量、痛经的发生率差异有统计学意义（P＜0.05）.成功妊娠241例（81.41％）,自然流产69例（28.63％）,且自然流产随妊娠妇女年龄增大而增高.≥40岁组再生育妇女妊娠率低于＜35岁组及35～39岁组,差异均有统计学意义（P＜0.05）.结论 地震极重灾区再生育妇女的月经异常、妊娠自然流产发生率明显升高,及时、有效的综合预防和干预,是提高其妊娠率、降低自然流产率的必要措施,并对提高灾区育龄妇女及配偶的长期健康水平,具有一定的积极意义.Objective To explore the effects of the earthquake disaster in Wenchuan birth again menstruation,pregnancy,and pregnancy outcome.Methods 296 women＇s fertility intention from our hospital obstetrics and Gynecology outpatients and inpatients between Jan 2011 and Dec 2012 were divided into 〈35 group （n=77）,35～39 years old group （n=163） and ≥ 40 group （n=56）,according to their ages.Statistical analysis was performed on the menstruation,pregnancy,pregnancy outcome and other factors.Results Statistically significant difference existed in 296 cases of reproductive women before and after the earthquake including the menstrual cycle,menstrual quantity and the incidence of dysmenorrheal.The pregnancy rate was 81.41 % （241/296）.The natural abortion rate was 28.63 % （69/241）,and the natural abortion rate of pregnant women increased with age.The re-birth of ≥ 4O age group was lower than that of 〈35years old group and 35～39 years old group （P〈0.05）.Conclusion For the influence of Sichuan Wenchuan earthquake,disaster area and fertile women menstrual abnormalities,increased the incidence of spontaneous abortion,comprehensive prevention and intervention timely and effectively is to improve the pregnancy rate,reduce the necessary measures of spontaneous abortion,and improve the health level of women of childbearing age long in disaster areas and their spouses.
