团队护理对腹腔镜胆囊切除术胆管损伤再手术患者的临床效果评价

摘　　要：目的探讨团队护理对腹腔镜胆囊切除术胆管损伤再手术患者的临床护理效果及预后。方法对84例腹腔镜胆囊切除术胆管损伤再手术患者,随机分为团队护理组42例,常规护理组42例。比较两组患者住院天数、并发症发生率、患者满意度和患者对健康知识的掌握情况,并随访患者的康复情况及拔管时间。结果住院天数、患者的满意度及患者对健康知识掌握情况方面,团队护理组均优于常规护理组(P<0.05),但两组并发症发生率无明显统计学差异(P>0.05)。结论团队护理临床效果优于常规护理,能够提高腹腔镜胆囊切除术胆管损伤再手术患者的预后。Objective To evaluate the team nursing for patients with reoperation resulted from bile duct injury in laparoscopic cholecystectomy.Methods 84 patients with reoperation resulted from bile duct injury in laparoscopic cholecystectomy were enrolled in this prospective study,and were randomly divided into two groups：team nursing group （n=42） and usual care group （n=42）.Length of hospitalization,the complication rate,patient＇s satisfaction,and patient＇s knowledge on health were compared.Kaplan-Meier method and Cox proportional hazard model were performed to evaluate the prognosis of patients.Results The length of hospitalization （days）,patient＇s satisfaction,and patient＇s knowledge on health were better in team nursing group than that in usual care group （P〈0.05）,whereas there was no significant difference for the complication rate （P=0.116）.The prognosis of patients in team nursing group was better （P=0.032） than in usual care group with odds ratio of 3.542 [95% confidence interval：1.213,10.338] （P=0.021）.Conclusion Team nursing was superior to usual care for patients with reoperation resulted from bile duct injury in laparoscopic cholecystectomy.
