雷公藤多甙联合糖皮质激素对肾病综合征调节性T细胞的影响和临床疗效分析

摘　　要：目的探讨雷公藤多甙联合糖皮质激素对肾病综合征患者调节性T细胞(Treg)的影响和对该病的临床疗效。方法将46例原发性肾病综合征患者分为治疗组和对照组,各23例。治疗组采用雷公藤多甙联合糖皮质激素治疗,对照组单用糖皮质激素治疗,比较两组临床疗效、24 h尿蛋白定量、血清胆固醇、血清肌酐、血清蛋白、Treg等指标的变化。结果治疗组总有效率明显高于对照组,24 h尿蛋白定量、血清胆固醇、血清肌酐明显低于对照组,Treg和Treg/T细胞比值、血清蛋白明显高于对照组,差异均有统计学意义(P<0.05)。结论雷公藤多甙联合糖皮质激素能通过上调Treg细胞比例、减少尿蛋白漏出、增加血清蛋白等,从而增加肾病综合征的临床缓解率。Objective To investigate the influence and clinical efficacy of Tripterygium glycosides combined with gluco-corticoid on regulatory T cells（Treg） of patients with nephritic syndrome（NS）. Methods A total of 46 patients with primary nephrotic syndrome（PNS） were divided into treatment group and control group,23 cases in each group. The treatment group adopt-ed Tripterygium glycosides combined with glucocorticoid,while the control group only used glucocorticoid. The clinical efficacy, 24 h urine protein,serum cholesterol,serum creatinine,serum albumin,Treg and so on in the two groups were compared. Results The overall response rate,ratio of Treg and Treg/T cells and serum albumin in the treatment group were obviously higher than those in the control group,while the 24 h urine protein,serum cholesterol and serum creatinine were prominently lower than those in the control group,and differences all had statistical significance（P〈0.05）. Conclusion Tripterygium glycosides combined with glucocorticoid can up-regulate the ratio of Treg cells,reduce leak of urine protein,increase serum albumin and so on,so as to improve the remission rate of NC in clinic.
