搐鼻散塞鼻联合四虫汤治疗顽固性偏头痛60例临床观察

摘　　要：[目的]观察搐鼻散塞鼻联合四虫汤治疗顽固性偏头痛疗效。[方法]使用前瞻性设计方法,对60例门诊患者使用搐鼻散(川芎、白芷、细辛、丁香、冰片各等分研细末),温开水调药末,制成小条状,医用纱布包裹塞鼻。四虫汤(全蝎研末吞服5g,蜈蚣研末吞服3g,水蛭研末吞服1.5g,土鳖虫、川芎各12g,白芍15g,生甘草6g,天麻、白芷各10g;头痛较剧加炒延胡15g,徐长卿30g;伴呕吐加姜半夏10g,陈皮6g;头痛偏于枕部加羌活、蔓荆子各10g;偏于左侧或右侧加柴胡、黄芩各6g;前额痛加知母10g,葛根15g;巅顶痛加藁本6g,吴茱萸3g),1剂/d,水煎300 mL,早晚口服。连续治疗20d为1疗程。观测临床症状、不良反应。治疗1疗程,判定疗效。[结果]治愈39例,显效15例,有效3例,无效3例,总有效率95.00%。[结论]搐鼻散塞鼻联合四虫汤治疗顽固性偏头痛,疗效满意,无副作用,值得推广。[Objective] To observe the nasal powder by nose plug combined with four worm Decoction in the treatment of refractory migraine. [Method] using a prospective design method,60 cases with convulsions nasal powder(Chuan Xiong,angelica,asarum,clove,borneol each attainment research to fine),warm boiling water adjustable powder,made into small strips,medical gauze wrap nasal plug. The four bug soup(grind swallow 5g scorpion,centipede grind swallow 3G,leech grind swallow 1.5g,woodlouse,Chuan Xiong 12 g,15g of white peony root,licorice root 6G,gastrodia tuber,dahurian angelica root 10g;have a headache more drama and fried Yan Hu 15 g,Cynanchum paniculatum 30g;vomiting and Pinellia 10 g,orange peel 6G headache;partial to the occipital and notopterygium root,chastetree fruit each 10g;partial to the left or right side plus bupleurum,baicalin the 6G;forehead pain and Anemarrhena 10 g,Pueraria 15g;parietal pain and Ligusticum 6G,Wu Zhuyu 3G),1 doses of /d,water simmer in water 300 mL,sooner or later oral. Continuous treatment with 20 d for 1 courses. observation of clinical symptoms,adverse reaction. 1 courses of treatment,the curative effect.[Result] 39 cases were cured,15 cases markedly effective,3 cases,3 cases ineffective,the total efficiency of 95%. [Conclusion] by nasal powder nasal plug combined with four worm Decoction in the treatment of refractory migraine,satisfied with the effect,no side effect,is worth promoting.
