王颖清热凉血治疗小儿紫癜性肾炎

摘　　要：王颖教授根据过敏性紫癜临床表现,将其归属于"肌衄"、"葡萄疫"等范畴,认为与《内经》中"溲血"、"溺血"类似,由先天禀赋不足,复感外邪而发病;初期多为外感风热,或湿热内蕴复感邪热,热毒内盛,灼伤血络,迫血妄行而为血尿,当止血为主,辅以清热凉血化瘀;后期热毒渐消,邪去正虚,治宜凉血消癍,补肾滋阴,扶正固本,恢复元气。尤其对滞热型体质,清热凉血贯穿治疗始终。王颖教授常使用小蓟饮子和犀角地黄汤,使血止而不留瘀,利尿而不伤阴,攻补兼施,扶正祛邪的同时不忘调整患儿体质,有效防止复发和减轻肾脏病理损害。同时重视随访,嘱患者注意平日起居保暖,预防感冒,适量运动。附病案一则。According to the clinical manifestations of allergic purpura, professor Wangying attributes it into the category of ＂muscle bleed＂＂grapes epidemic＂ which is similar with ＂urinate blood＂＂drowning blood＂ in neijing.Hereditary deficiency , plus external pathogenic factor can lead to this disease.In initial stage, infected by external wind heat or dampness with heat fixed in the body besides pathogenic heat. The heat can transform into boiled energy which can destroy the vessels of blood.The leak of blood forms the blood urine. Treatment basically on hemostasis, by cleaning heat cooling blood and removing blood stasis as accessory cures.While later stage with pathogenic heat toxic disappeared, vital Qi weakens. Transform the treatment into cooling blood for resolving macula, nourishing kidney Yin, strengthening vital Qi to restore primordial Qi. Especially for those stagnated heated constitution patients, cleaning heat and cooling blood throughout the treatment .Professor wangying applies xiaojiyinzi and xijiaodihuangtang, which results in hemostasis without blood stasis and diuresis without Yin loss.Reinforcement and elimination in combination. Take patients＇ constitution adjustment into consideration while strengthening vital Qi and eliminating pathogen.Only in this way can prohibit the recurrence and mollify kidney pathological damage. Simultaneously pay attention to follow-up visit, give instructions below ： keep warm and prevent cold in daily life. Get enough exercise. Enclose a medical record with the paper.
