摘　　要：目的探讨病毒性肝炎患者血清甲状腺素(T4)、铜蓝蛋白(CP)、丙氨酸氨基转移酶(ALT)和凝血酶原时间(PT)水平变化与肝功能损害程度的相关性及其临床意义。方法将所有病例分成慢性肝炎组、重型肝炎组和肝硬化组,分别采用化学发光法、散射免疫比浊法、连续监测法和凝固法分别检测T4、CP、ALT和PT。结果各病例组ALT、PT和T4与正常对照组比较,差异均具有统计学意义(P<0.05);肝硬化组ALT明显低于其他2个病例组(P<0.01);各病例组PT之间差异均具有统计学意义(P<0.05);慢性肝炎组T4高于其他病例组(P<0.05);重型肝炎组CP低于正常对照组及其他病例组(P<0.05),但慢性肝炎组和肝硬化组CP与正常对照组之间差异无统计学意义(P>0.05)。结论联合检测T4、CP、ALT和PT对病毒性肝炎的早期和鉴别诊断、病情判断以及疗效和预后评估有重要参考价值。Objective To study the clinical significance of the change of thyroxine（ T4）,ceruloplasmin（ CP）,L- alanine aminotransferse（ ALT） and prothrombin time（ PT） in sera of patients with viral hepatitis,and the correlation between them and the damage degree of liver function. Methods All patients were divided into chronic hepatitis,severe hepatitis and liver cirrhosis group. T4,CP,and ALT in serum and PT in plasma were detected by chemiluminescence immunoassay,nephelometry immunoassay,continuous monitoring assay and coagulation method,respectively. Results There was statistically significant difference in T4,ALT and PT between patients and normal control（ P 0.05）. ALT in liver cirrhosis group was obviously lower than that in other patients（ P 0. 01）. There was statistically significant difference in PT groups（ P〈0. 05）. T4 in chronic hepatitis group was higher than that in other patients（ P〈0. 05）. CP in severe hepatitis group was lower than that in normal control and other patients（ P〈0. 05）. Compared with the normal group,the difference in CP in chronic hepatitis group and liver cirrhosis group was not significant（ P〉0. 05）. Conclusion The combined detection of T4,CP,ALT and PT is very important in early diagnosis,differential diagnosis of viral hepatitis,judgement of patients＇ condition,evaluation of drugs＇ effect and prognosis to patients with viral hepatitis.
