摘 要:目的探讨糖尿病及腹膜通透性对腹膜透析病人腹透液清蛋白丢失量的影响。方法选择我院透析时间达6个月的腹膜透析病人68例,其中糖尿病病人38例,非糖尿病病人30例。根据腹膜平衡实验,将病人分为两组,高转运与高平均转运的病人36例为高通透性组,低转运与低平均转运病人32例为低通透性组。同时检测各组病人24h腹透液清蛋白及血浆清蛋白、血红蛋白、血尿素氮、肌酐水平。结果高通透性组较低通透性组腹透液清蛋白丢失量明显增多(t=2.118,P<0.05),血浆清蛋白明显下降(t=2.374,P<0.05)。糖尿病组较非糖尿病组腹透液清蛋白丢失量明显增多(t=2.887,P<0.05),血浆清蛋白明显降低(t=2.278,P<0.05)。结论糖尿病与腹膜高通透性均可造成病人腹透液清蛋白丢失量增加,从而导致病人营养不良发生率升高。Objective To investigate the influence of diabetes and peritoneal permeability on loss of protein in peritoneal dialysis solution. Methods Sixty-eight patients who underwent peritoneal dialysis up to six months were enrolled in this study, of whom, 38 were diabetic and 30 non-diabetic. According to peritoneal equilibration test (PET) : 36 with high transport and high average transport were classified as high-permeability group, and 32 with low transport and low-average transport classified as lowpermeability group. Albumin in 24 h-dialysis solution, plasma albumin, hemoglobin, creatinine and blood urea nitrogen were measured. Results Peritoneal albumin loss in high-permeability group increased (t=2.118,P〈0.05) and plasma albumin decreased (t = 2.374 ,P〈0.05) versus the low-permeability group. Peritoneal albumin loss in the diabetic group was much more than that in the non-diabetic group (t=2.887,P〈0.05), and the plasma albumin declined (t=2.887,P〈0.05). Conclusion Both diabetes and high permeability of peritoneum may lead to increasing of loss of albumin in peritoneal dialysis fluid, resulting in the elevation of incidence of malnutrition in patients undergoing peritoneal dialysis.
