摘  要:目的:探讨体外受精-胚胎移植(IVF-ET)过程中血清及卵泡液催乳激素(PRL)的变化规律及其意义。方法:检测38例接受IVF-ET患者治疗前(基础日)、治疗中(Gn 6日、HCG日、取卵日)血清PRL水平及取卵日卵泡液(左、右卵巢)PRL水平,分析变化规律及其与获卵、受精、胚胎质量及妊娠情况的相关性。结果:1PRL在取卵日左、右卵巢卵泡液的浓度明显高于基础日、Gn 6日、HCG日血清水平(P<0.05)。2PRL的血清浓度从卵泡期开始上升,在HCG日达峰,约为基础日浓度的2.43倍,取卵日稍下降,Gn 6日、HCG日和取卵日的血清PRL浓度均显著高于基础日血清PRL浓度(P<0.05)。3HCG日血清PRL浓度与回收卵数、成熟卵数呈正相关(P<0.05);取卵日血清PRL浓度与回收卵数、成熟卵数、受精数、优胚数均呈正相关(P<0.05);其余指标之间无显著相关性。4妊娠组与未妊娠组在IVF-ET过程中基础日、Gn 6日、HCG日、取卵日血清和左、右卵巢卵泡液中的PRL值比较,差异均无统计学意义(P>0.05)。结论:垂体降调节及控制性超促排卵过程对垂体的PRL分泌无显著影响,HCG日、取卵日的血清PRL的浓度与获卵、受精及胚胎质量有一定的相关性。IVF-ET与正常月经周期存在相似的一过性PRL升高过程,但未影响妊娠结局。Objective:To investigate the change of prolactin in serum and follicular fluid in the course of in vitro fertilization and embryo transfer(IVF-ET). Methods:The serum level of prolactin at different time points before and in the course of IVF-ET and the level in follicular fluid on the day of oocyte retrieval of 38 cases were examined. The change trend of prolactin and its correlations to the number of retrieved oocytes fertilized oocytes, and superior embryo,and pregnancy outcome were analyzed. Results:①The Prolactin level in follicular fluid in both sides was significantly higher than that in blood serum at different time points( P 〈 0.05). ②The serum prolactin level increased from the follicular phase, and reached a maximum on the day of HCG injection, then dropped down slightly on the day of oocyte retrieval. All the serum levels at the following monitoring time were significantly higher than that before treatment( P 〈0. 05). ③The serum prolactin level on the day of HCG injection was positively correlated to oocytes number and ovium number( P 〈 0.05), and the serum prolactin level on the day of oocyte retrieval was positively correlated to oocytes number,ovium number,fertilized oocytes number and superior embryo number( P 〈0. 05). There was no significant correlation among other factors. ④There were no significant differences of prolactin levels in serum or follicular fluid at any time points between the pregnant and the non-preg- nant group( P 〉 0.05). Conclusions: The pituitary down-regulation and the course of controlled ovarian hyperstimulation had no significant influence on the pituitary secretion of prolactin. The serum levels on the days of HCG injection and oocyte retrieval were related to the number of retrieved oocytes, fertilization and embryo quality.There was a transient increase of prolactin in the course of IVF-ET similar to normal menstruation, but it did not affect the pregnancy outcome.
