外侧柱单钢板固定治疗肩胛骨体部骨折的初步疗效

摘　　要：目的 探讨外侧柱单钢板治疗肩胛骨体部骨折的初步疗效. 方法 回顾性分析2012年5月至2013年8月采用外侧柱单钢板治疗明显移位的15例肩胛骨体部骨折患者资料,男12例,女3例;年龄22 ～ 50岁,平均38.5岁，记录患者返回正常工作或活动的时间、并发症发生情况以及末次随访时肩关节Constant评分和臂肩手功能障碍评分（DASH）， 结果 所有患者术后获8～14个月（平均11.5个月）随访，骨折临床愈合时间为6～12周,平均7.5周.所有患者未出现伤口感染、血肿、肩胛上神经损伤、内固定物松动或断裂、肩关节持续性疼痛等并发症.所有患者术后返回正常工作或活动的时间为2.5～4.0个月（平均3.5个月），末次随访时肩关节Constant评分平均为79.5 ～ 95.0分（平均90.0分）,其中优14,良1例;DASH评分为1.5～11.0分（平均6.5分），结论 外侧柱单钢板固定治疗肩胛骨体部骨折可以获得满意复位、相对稳定固定及良好的近期疗效。Objective To report the preliminary outcomes of scapula body fractures treated by single plating at the lateral border.Methods In the period from May 2012 to August 2013,15 fractures of displaced scapula body were treated at our department by open reduction and internal fixation with single plating at the lateral border.They were 12 men and 3 women,aged from 22 to 50 years （mean,38.5 years）.The therapeutic outcomes were assessed in terms of the time to normal work and activity,complications and shoulder functions by the Constant score and Disabilities of the Arm,Shoulder and Hand （DASH） score at the last follow-up.Results All the patients were followed up from 8 to 14 months （average,11.5 months）.All fractures healed after 6 to 12 weeks （average,7.5 weeks）.No such complications were observed as infection,hematoma,suprascapular nerve injury,implant failure or persistent shoulder pain.All patients returned to normal work or activity after a mean time of 3.5 months （from 2.5 to 4.0 months）.At the last follow-up,14 cases were excellent and one was good; the average Constant score was 90.0 points （from 79.5 to 95.0 points）; the average DASH score was 6.5 points （from 1.5 to 11.0 points）.Conclusion Open reduction and single plating via the lateral border approach for fractures of displaced scapula body can lead to satisfactory reduction,relatively stable internal fixation and good short-term outcomes.
