耳部瘢痕疙瘩核心切除及瘢痕组织瓣修复并及时放疗的方法介绍

摘　　要：目的：探讨切除耳部瘢痕疙瘩的手术方法和及时放疗联合应用的疗效.方法：对1 34例耳部瘢痕疙瘩病例采取不同的手术方法,特别对较大的瘢痕疙瘩采用了瘢痕疙瘩核心切除,保留部分瘢痕皮肤形成组织瓣的方法修复缺损,术后及时放疗5天,采用4- 6Mev电子线照射,每次3- 4Gy,总剂量15- 20Gy.结果：96.27％的效果满意,3.73％部分残留及复发.结论：切除瘢痕疙瘩核心,用部分保留瘢痕皮肤形成组织瓣的修复方法,术后外观满意,及时放疗可以预防或减少复发,方法简便实用,值得临床推广应用.Objective To investigatethe surgery methods of the ear＇s keloid,and the effect of combining the radiation therapy in time. Methods To do different surgeries treating ear＇s keloid, but for the bigger ear＇s keloid,treating with the keloid skin flap which cutting off the core. Combined the post- operation radiation therapy in 5 days.Using 4-6Mev X-ray, do the therapy with 3-4Gy each time, and the total dosage between 15 to 20Gy. Results 96.27% of patients are satisfied with the consequences,but 3.73% are recurrence. Conclusion Using keloid skin flap fixed the defect without the core of the keloid, which combined with post-operation radiation therapy in time could prevent and reduce the recurrence.Most of patients feel satisfied about the appearance after surgery.This method is simple and practical, and worth to promoting.
