摘 要:目的 比较糖调节受损(IGR)及初发2型糖尿病(T2DM)患者与健康人外周血脂肪细胞型脂肪酸结合蛋白4(FABP4)及人第10号染色体缺失的磷酸酶及张力蛋白同源物(PTEN)的蛋白表达,探讨两者可否作为糖代谢紊乱早期的生化指标.方法 2013年1月至12月,收集初发T2DM(分为肥胖组34例及非肥胖组31例)患者共65例、IGR患者32例及健康对照组30例.IGR及T2DM组中男48例,女49例,年龄(47± 11)岁.对照组中男14例,女16例,年龄(41±11)岁.采用酶联免疫吸附法检测受试者外周血浆中FABP4与PTEN的蛋白表达水平,同时检测血糖、血脂等指标.多组间变量比较采用单因素方差分析,两变量间相关分析采用Pearson相关分析,多元逐步回归分析方法用于确定胰岛素抵抗的独立影响因子.结果 (1)对照组、IGR组、非肥胖糖尿病组和肥胖糖尿病组FABP4浓度分别为(3l±4)、(34±5)、(34±4)、(34±5) ng/L.与对照组比较,IGR组、非肥胖糖尿病组和肥胖糖尿病组的FABP4浓度升高(t=2.057、2.369、3.094,均P<0.05).(2)对照组、IGR组、非肥胖糖尿病组和肥胖糖尿病组PTEN浓度分别为(2.9±0.5)、(3.8±0.6)、(3.7±0.7)、(3.8±0.5)μg/L.与对照组比较,IGR组、非肥胖糖尿病组和肥胖糖尿病组的PTEN浓度升高(t=6.049、5.509、5.798,均P<0.05).(3)相关分析显示FABP4与PTEN呈正相关(r=0.361,P<0.01);经多元逐步回归分析发现,甘油三酯、体质指数、FABP4 、PTEN是HOMA-IR的独立相关因素.结论 FABP4及PTEN有可能作为糖代谢紊乱的早期生化检测指标,并与T2DM患者胰岛素抵抗可能相关.Objective To compare the plasma protein expressions of fatty acid binding protein 4 (FABP4) and phosphatase and tensin homolog deleted on chromosome ten (PTEN)in patients with impaired glucose regulation(IGR) and newly-diagnosed type 2 diabetes mellitus(T2DM),and investigate whether they can be the early biochemical markers of glucose metabolism disorders.Methods From January to December 2013,65 newly-diagnosed T2DM patients(34 obese patients and 31 non-obese patients),32 impaired glucose regulation(IGR) patients and 30 healthy subjects were included in this study.Among the IGR and T2DM groups,48 were male,49 female and mean age was (47± 11) years,while in healthy subjects,14 were male,16 female and mean age was (41± 11) years.Plasma protein expression levels of FABP4 and PTEN were measured using enzyme-linked immunosorbent assay (ELISA),and other indicators such as fasting glucose and lipids were detected in the meantime.One-way ANOVA was used to compare quantitative data among groups.Pearson correlation analysis was used for correlation analysis between the two variables,and Multiple stepwise regression analysis was used to determine the independent factors of HOMA-IR.Results (1)The plasma concentration of FABP4 in the normal control group,IGR group,non-obese and obese groups of diabetes was (31±4),(34±5),(34±4) and (34±5)ng/L,respectively.Plasma FABP4 concentrations increased in the IGR group,non-obese and obese group of diabetes compared with the control group(t value was 2.057,2.369 and 3.094,respectively,all P<0.05).(2) The plasma concentration of PTEN in the normal control group,IGR group,non-obese and obese group of diabetes was (2.9±0.5),(3.8±0.6),(3.7±0.7) and (3.8±0.5) μg/L,respectively.Compared with control group,the plasma concentrations of PTEN increased in IGR group,non-obese and obese groups of diabetes (t value was 6.049,5.509 and 5.798,respectively,all P<0.05).(3) The levels of FABP4 and PTEN w
