买朱尼·阿扎拉克蜜膏对膝骨关节炎患者的疗效及其机制研究

摘　　要：目的：观察维药买朱尼·阿扎拉克蜜膏对膝骨关节炎患者的治疗作用及对血清中基质金属蛋白酶-1、基质金属蛋白酶抑制剂-1、白细胞介素1β（IL-1β）的影响。方法：纳入2013年2月-2014年2月就诊于新疆医科大学附属中医医院骨科诊断为膝关节骨性关节炎患者80例。将符合标准的膝骨关节炎患者按随机数字表分成两组。基础治疗选用双氯芬酸钠缓释胶囊1次/天,1粒/次,连服4周。治疗组在基础治疗上口服买朱尼·阿扎拉克蜜膏四周,2次/日,10克/次;对照组在基础治疗上加用包装、剂量与买朱尼·阿扎拉克蜜膏一样的安慰剂,服用方法同治疗组。结果：两组患者未出现脱落及失访,其中对照组1例出现消化道反应,调整用药后未再发;两组治疗前Lysholm功能评分差异无统计学意义;两组治疗后与治疗前比较差异显著（P〈0.05）,两组患者临床症状均明显缓解,膝关节功能得到改善;两组患者治疗后Lysholm评分比较（P〈0.05）,治疗组优于对照组;总体有效率分别为95.9%、80.0%,差异有统计学意义（P〈0.05）,治疗组优于对照组;治疗后治疗组患者血清基质金属蛋白酶-1、IL-1β含量均低于治疗前（P〈0.05）,基质金属蛋白酶抑制剂-1高于治疗前（P〈0.05）;对照组上述各指标差异均无显著性（P〉0.05）;治疗后两组间基质金属蛋白酶抑制剂1、白细胞介素1β含量差异具有显著性（P〈0.05）;随访期间安全性指标正常。结论：维药买朱尼·阿扎拉克蜜膏在降低关节内炎性物质分泌的同时,通过对软骨细胞外基质降解的调控,进而对骨关节炎的进展有更好的延缓作用,通过对关节内胶原的保护,促进了破坏的关节软骨面的自我修复。To observe the Uygur Medicine Maizhuni bee grease in the treatment of patients with knee osteoarthritis and on serum matrix metalloproteinase-1, matrix metalloproteinase inhibitor-1, interleukin 1 β （IL-1 β） effect. Methods： In 2013 February - 2014 February visit to the traditional Chinese Medicine Hospital Affiliated to Xinjiang Medical University Department of orthopedics diagnosis for 80 cases of patients with knee osteoarthritis. Will meet the standard of the knee osteoarthritis patients are randomly divided into two groups, namely, di- vided into treatment group, Shuang Hao divided into the control group. Basic therapy Diclofenac Sodium Sustained Release Capsules 1 times / day, 1 pills each time, and even served 4 weeks. The treatment group in the basic treatment of oral maizhuni bee grease,2 times/day, 10 g/ times; the control group was treated with packaging, dose and maizhuni bee grease,like placebo, taking the method with the treatment group. Results： Two patients did not show off and lost to follow-up, 1 patients in control group appeared gastrointestinal reaction, adjust medication without recurrence; Lysholm score had no significant difference between the two groups before treatment （ P 〉0. 05 ） ; the difference between the two groups before and after treatment were significantly （ P 〈 0.05）, the two groups the clinical symptoms of patients were relieved, knee joint function improvement; Lysholm score after treatment in two groups, there was significant difference （P 〈 0.05 ）, the treatment group than the control group; the total effective rate were 95.9% ,80%, the difference was statistically significant （ P 〈 0.05）, the treatment group than the control group; treatment after treatment, serum levels of matrix metaUoproteinases -1, IL-1 β levels were lower than those before ther- apy （P 〈 0.05 ）, matrix metalloproteinase inhibitor -1 was higher than that before treatment （P 〈 0,05 ） ; group differences in the above in- dexes control were not significant �
