慢性心力衰竭患者LDH与血浆BNP及中医证型相关性研究

摘　　要：目的:探讨慢性心力衰竭患者LDH与血浆BNP水平相关性,以及LDH与中医证型的相关性。方法:将慢性心力衰竭患者300例按照《中药新药临床研究指导原则(试行)》分型,并检查记录BNP、LDH数值,通过统计学方法探讨其相关性。结果:LDH与血浆BNP相关性探讨,r=0.167,P<0.01,提示LDH与BNP呈弱正相关;LDH与中医证型相关性探讨,经秩和检验χ2=25.272,P<0.00 1,提示各证型之间差异有统计学意义,并且按照各证型秩均值,随LDH的降低,证型依次为:阴竭阳脱、痰饮阻肺、心肾阳虚、气虚血瘀、阳虚水泛、气阴两亏、心肺气虚。结论:LDH与BNP以及中医证型存在一定相关性,可作为心衰程度判断以及中医证型判定的客观参考数据。Objective： To explore the relationship between the levels of LDH and plasma BNP of the patients with chronic heart failure （CHF） and the connections of LDH to TCM patterns. Methods： All 300 patients were grouped according to guiding principle of clinical research on new drugs of Chinese medicine （Trial）, BNP and LDH of all the patients were examined and recorded, the relativity was explored by statistical method. Results： The relativity between LDH and plasma BNP was explored and showed, r=0.167, P〈0.01, which suggested that it was a a weak positive correlation between LDH and BNP; the discussion on the correlation between LDH and TCM pat- terns, testified by rank test, indicated X^2=25.272, P〈0.001, it presented that the differences had statistical meaning between the patterns, the order of patterns were： Yin-depletion and Yang-collapse, phlegm-fluid obstructing the lung, Yang deficiency of heart-kidney, Qi deficiency and blood stasis, Yang-deficiency and edema, dual depletion of Yinyin, Qi-deficiency of heart-lung according to mean rank as LDH lowered. Conclusion： LDH shows a certain connection to BNP and TCM patterns, which could be taken as subjective reference for judging the degrees of heart failure and TCM patterns.
