摘 要:目的:观察大黄有效部位(Rep)对慢性环孢素肾病(CCN)的作用。方法:雄性Wistar大鼠36只,随机平均分为:正常对照组(VH组),CCN模型组(MX组),贝那普利阳性对照组(YDY组),Rep干预1组、干预2组、干预3组(DY1、DY2、DY3)。在用药前和用药后的每周最后一天测量大鼠体质量并采血测定血尿素氮(BUN)和血肌酐(Scr)。实验末处死大鼠,HE染色、PAS染色及Masson染色,光镜观察肾组织病理变化,通过半定量积分评估各组小动脉透明样变性、肾间质纤维化(RIF)情况。同时取肾组织行电镜检查观察超微结构变化。结果:1给予环孢霉素A后,MX组大鼠体质量下降;4周后所有Rep干预组及YDY组体质量均高于MX组,而BUN、Scr水平均低于MX组。2光镜结果:VH组大鼠肾组织结构正常;MX组大鼠呈现典型的CCN改变;而各Rep干预组及YDY组大鼠的CCN病变明显减轻。3电镜结果:VH组结构正常;MX组大鼠肾小球、肾小管基底膜增厚,系膜基质增多,足细胞足突广泛融合、微绒毛稀疏,间质见较多胶原纤维;Rep各干预组及YDY组病变均减轻。结论:用Rep干预用药,CCN病变可明显减轻,早期干预效果更好;以Rep预防CCN病变,其效果优于贝那普利。Objective: To observe the effect of Rubarb's Effective Parts( Rep) on chronic cyclosporine nephropathy( CCN). Methods: 36 SPF Wistar rats were divided into 6 groups randomly: normal control group( VH); CCN model group( MX); benazepril positive control group( YDY); three Rep intervention groups( DY1,DY2,DY3). Before and after medication,rat's body weight was measured at the last day in every week,simultaneously,venous blood was collected to measure blood urea nitrogen( BUN) and serum creatinine( Scr).Harvested kidney tissues were stained with HE,PAS and Masson. Pathological changes were observed. The extend of hyaline degeneration of small arteries and renal interstitial fibrosis( RIF) changes were assessed by a semiquantitative score. The ultrastructural changes of kidney tissues were alsoobserved. Results: ① After CsA administration,the weight of rat in MX group decreased; It was lower than that of rat in YDY and all Rep intervention groups. However,the rat BUN and Scr level of MX group was higher than that in the other groups.② Light microscopy showed normal structure of kidney tissues in VH group. Typical CCN was found in MX group. The CCN changes of YDY and Rep intervention groups significantly reduced. ③ The ultrastructure was normal in VH group. In MX group,it was as follows: glomerular and tubular basement membrane thickened,chromatin thickened and concentrated,pyknosis,collagen fiber increased in interstitium. Pathological changes of YDY and Rep intervention groups significantly lightened. Conclusion: Rep intervention could significantly reduce CCN changes. Early intervention,better result would be obtained. Rep intervention showed better effect than benazepril.
