二甲双胍肠溶片3种联合用药方案治疗2型糖尿病的数据包络分析

摘　　要：目的:评价二甲双胍肠溶片3种联合用药方案治疗2型糖尿病的数据包络分析。方法:60例患者分为3组,分别给予二甲双胍肠溶片联合非磺脲类药物(瑞格列奈片,A组)、二甲双胍肠溶片联合基础胰岛素组(甘精胰岛素,B组)、二甲双胍肠溶片联合预混胰岛素组(精蛋白重组人胰岛素M30,C组)治疗1年,期间每3个月随访1次。以医疗成本为投入,空腹血糖(FBG),餐后2 h血糖(PBG2h),糖化血红蛋白(HbA1c%),质量调整生命年(QALYs)为产出,使用数据包络分析(DEA)对投入和产出进行经济学综合评价。结果:C组的临床效果、生活质量评分优于A组和B组。数据包络分析结果显示C组投入和产出达到最优,A组、B组投入成本偏高且临床疗效有待提高,QALYs值A组有待提高B组获得的已达到理想值。结论:二甲双胍肠溶片联合预混胰岛素方案具有更好的临床效果和经济学效果。OBJECTIVE To evaluate the data envelopment analysis （DEA） of metformin hydrochloride enteric-coated tablets in combination with different drugs in the treatment of type 2 diabetes. METHODS A total of 60 patients were enrolled and randomized to Group A （metformin ＋ repaglinide, 20 cases）, Group B （metformin ＋ insulin glargine injection, 20 cases） or Group C （metformin ＋ protamine zinc recombinant human insulin M30, 20 cases）. The DEA of the 3 groups was evaluated after treatment for 1 year with elinical randomized trial and followed up every 3 months. The medical cost was taken as the input, fasting blood glucose （FBG）, 2h postprandial blood glucose （PBG2h）, glycosylated hemoglobin （HbAlc）, quality adjusted life years （QALYs） as the output, and using data envelopment analysis （DEA） for comprehensive evaluation of the economics of input and output. RESULTS Clinical effect and QALYs of Group C were better than those of Group A and B. DEA results showed that the input and output of Group C achieved opti- mization. The investment costs of Group A and B were higher and the clinical curative effects to be improve＆ QALYs of Group A nee- ded to be improved; while that of Group B had reached the ideal value CONCLUSION Metformin enteric coated tablets combined with premixed insulin therapy has a better clinical effect and economic effect.
