摘 要:目的:探讨黄连溶液对早期(Ⅰ、Ⅱ期)压疮缺血再灌注损伤中缺氧诱导因子-1α(HIF-1α)、血管内皮生长因子(VEGF)含量变化的影响。方法:SD大鼠40只随机分为4组,正常组、模型组、酒精组、黄连溶液组,每组10只。正常组大鼠备皮后不做任何处理,余各组采用埋置铁片外加磁铁施压的方法复制压疮模型;酒精组、黄连溶液组给予相应药物外涂。5 d后处死大鼠,取压疮创面皮肤肌肉组织,观察病理组织学变化,检测组织中HIF-1α、VEGF因子的表达。结果:病理组织学观察发现,模型组肌纤维萎缩,肌间隙间水肿明显,伴有大量炎性细胞浸润;酒精组镜下变化与模型组相似,而黄连溶液组细胞及组织水肿较模型组减轻、炎症细胞明显减少。组织中HIF-1α、VEGF表达显示,模型组较正常组增多,黄连溶液组干预后HIF-1α、VEGF表达较模型组进一步升高(P<0.05),而酒精组与模型组无明显差异(P>0.05)。结论:黄连溶液具有减轻早期压疮缺血再灌注损伤的作用,其机制可能与通过促进创伤组织HIF-1α及其下游因子VEGF的表达有关。Objective: To study the effects of Rhizoma Coptidis solution on expressions of hypoxia inducible factor-1α( HIF-1α) and vascular endothelial growth factor( VEGF) on early stage( I,II) of pressure ulcers with ischemia reperfusion injury in rats. Methods: Forty SD rats were randomly divided into 4 groups: normal group,model group,alcohol group,and Rhizoma Coptidis solution group,with 10 rats in each group. After skin preparation,the rats in normal group weren't given any treatment; rats in the other groups were replicated pressure ulcer models by embedding sheet iron inside together with outside magnet pressure method. Alcohol group and Rhizoma Coptidis solution group were coated with corresponding drugs. All the rats were sacrificed after 5 days,the skin and muscle tissues of the pressure ulcers were harvested to observe the histopathological changes and detect the expressions of HIF-1 α and VEGF. Results: Histopathological observation showed that,in model group,the muscle fiber atrophied and there was an obvious muscle interstitial edema,accompanied by a large number of inflammatory cell infiltration; the microscopic changes in the alcohol group were similar to those of the model group; while in the Rhizoma Coptidis solution group,the degree of cells and tissues edema were slighter than those of the model group,the number of inflammatory cells were significantly reduced compared with the model group. There was more expressions of HIF-1α and VEGF of tissues in the model group than that of the normal group,and the HIF-1 α and VEGF expressions were further increased after intervened by Rhizoma Coptidis solution,it was higher than that of the model group( P 〈0. 05),while there was no significant difference between the alcohol group and the model group( P 〈0. 05). Conclusion: Rhizoma Coptidis solution can reduce the progress of the early pressure ulcer injury. Its mechanism may be association with regulating the expressions of HIF-1α and its downstream factor VEGF.
