摘 要:目的:厘清公立医院治理制度改革利益相关方的社会期望和部门目标,对比各利益相关方的社会公益和人员的个人利益,明确各利益相关方在公立医院治理制度改革中的立场。方法:采用文献综述法和专家访谈法,依据行政伦理学理论和罗素的政治欲望学说分析各利益相关方的社会期望和组织目标。结果:公立医院的治理制度改革的各个利益相关方影响政策的制订和实施,又受政策结果的影响。组织机构本身作为政策的影响者和利益的博弈者是抽象的,它只能通过有血有肉有思想意识的人发挥作用。因此,组织中的群体或个人才是真正的利益相关者和政策影响者,他们的行为才是具体而可观察。结论:目前对公立医院改革种种困难和问题的讨论存在一个通病,就是把组织机构跟组织群体生命混为一谈,把组织的书面职能当成组织群体的实际意愿。在我国行政管理体制的约束和传统行政文化的影响下,各利益相关方对政策的影响力与其说是国家组织法和机关、事业单位职能的体现,不如可以认为是部门最高行政官员和管理集体对政策的理解及个人意志、能力的体现。制定针对性的绩效考核办法,引导个人目标向改革目标靠拢,是公立医院治理制度改革成功的关键。Objective: To clarify the social desirability and department goal of stakeholders of governance system reform in public hospitals, compare the social profits and personal profits, clear the standpoint of different stakeholders of the governance system re form in public hospitals. Methods: Using the literature review method and expert interviews method to analyze the social desirability and department goal of the stakeholders based on the administrative ethics theory and Russell's political desire doctrine. Results: The stakeholders influenced the tbrmulation and implementation of reform policy and were affected by the policy at the same time. As the influencem of the policy and the gamer of the interest is abstraet, organization itself could only come into play by fresh, con- scious and activity person. Therefore the groups and personages in the organization were the real stakeholders and policy decision mak- ers, whose behavior were specific and observable. Conclusion: There was a common failing in the discussion about the difficuhies and problems of public hospital reform which confused the organization life with the organization group life, mistaken the organization function in written form with organization group's practical will. Under the influence of administrative control system and traditional administrative cuhure of our country, instead of saying that the influence of stakeholders were the reflect of the country organic law and public institution function. In fact, it could be considered as the reflect of understanding and individual will and capacity of de parlment's highest administrative officer and management collectivity. Formulating specific performance appraisal method to guide individual goal draw close to reform goal was the key to the success of public hospital governance system reform.
