摘  要:目的了解2006年-2010年临床感染病原菌的分布及耐药变化情况,为临床合理用药提供依据。方法药敏试验采用K-B法,WHONET5.0软件进行数据统计分析。结果共分离病原菌2 356株,其中革兰阴性杆菌1 364株,占57.9%,排前4位的为大肠埃希菌、铜绿假单胞菌、肺炎克雷伯菌、黏质沙雷菌;碳青霉烯类抗菌药物对肠杆菌具有高度抗菌活性。非发酵革兰阴性杆菌对碳青霉烯类抗菌药物也具有高度抗菌活性。革兰阳性球菌992株,占42.1%,万古霉素对MRCNS、MRSA具有高度抗菌活性,对青霉素、红霉素、庆大霉素等的耐药率偏高(>90.0%)。链球菌主要由对四环素耐药高的肺炎链球菌及其他链球菌组成。结论碳青霉烯类抗生素对革兰阴性杆菌,万古霉素、替考拉宁对革兰阳性球菌仍为抗菌活性最强的抗菌药物,医院细菌耐药呈增长趋势,加强对多重耐药的耐药性检测,对于合理选用抗菌药物十分重要。Objective To study the clinical distribution and drug resistance variability of pathogens from nisocomial infection isolated from 2006 to 2010 and to provide a basis for clinical rational use of drugs. Methods The drug sensitive test was conducted by k- b method and,and the data was analyzed statistically by WHONET 5. 0 software. Results Totally 2 356 strains pathogenic bacteria were separated,including gram negative bacilli 1 364 strains,accounting for 57. 9%,the former four was escherichia coli,pseudomonas aeruginosa,klebsiella pneumoniae,Serratia marcescens bacteria; carbapenems remained was the most active against stains of enterobacteriaceae bacteria; carbapenems remained most active against stains of nonfermentative gram- negative bacilli too. Gram positive coccus were 992 strains,accounting for 42. 1%,vancomycin remained most active against stains of MRCNS, MRSA. To penicillin, erythromycin, gentamycin and so on, drug resistance rate was high( 90.0%). Streptococcus mainly consists of drug resistance to tetracycline of streptococcus pneumoniae and other streptococcu. Conclusion Carbapenems to gram- negative bacillus; vancomycin,and teicoplanin to gram positive coccus still had the strongest antimicrobial activity of antibacterial drugs. Bacterial resistance was increased year by year in hospital,so strengthening the multiple drug resistance testing for the reasonable selection of antibiotic medicines is very important.
