电针结合推拿对气滞血瘀型原发性痛经患者血清PGF2α及β-EP含量的影响

摘　　要：目的:通过观察、比较电针结合推拿与痛经灵颗粒两种不同治疗方法对气滞血瘀型原发性痛经的临床疗效及对血清前列腺素(PGF2α)、β‐内啡肽(β-EP)的影响,寻求一种更好治疗该病的方法。方法:将60例气滞血瘀型原发性痛经患者随机分成治疗组和对照组,每组30例。治疗组采取电针结合推拿的治疗方法,对照组采取口服痛经灵颗粒的治疗方法。观察两组临床疗效和治疗前后血清PGF2α、β-EP水平的变化。结果:(1)治疗前与治疗后3个月经周期内,两组的VAS评分均呈下降趋势,但两组组间比较,治疗组变化比对照组显著,差异有统计学意义(P<0.05)。(2)治疗前,两组血清中PGF2α及β-EP含量与正常对照组比较显示,血清的PGF2α水平均显著升高,β-EP水平均显著降低(P<0.05)。治疗后,两组血清中PGF2α水平均显著降低,β-EP水平均显著升高(P<0.05)。组间比较:差异不明显,无统计学意义(P>0.05)。(3)两组的临床疗效比较:治疗组总有效率为90%,对照组为80%,两组相比治疗组显著高于对照组,差异有统计学意义(P<0.05)。结论:(1)电针结合推拿和痛经灵颗粒对血清中PGF2α及β-EP含量均有调整作用,但二者作用差异不明显。(2)两种治疗方法对气滞血瘀型原发型痛经均有治疗作用,但二者相比,电针结合推拿的治疗方法略优于痛经灵颗粒。Objective：To observe the variety of PGF2α and β -EP and compare the therapeutic effect of Tongjinglin granules and combination therapy of electroacupuncture and massage on primary dysmenorrheal of Qi stagnation and blood stasis type in patients at a specified time and to find out a better way which can make the curative effect last longer. Methods ： We randomly divided 60 patients into treatment group（ 30 cases） and control group （30 cases）. Patients in treatment group received the combination therapy of electroacupuncture and massage, while patients in control group received Tongjingling. We used visual analog scale （VAS） ,PGF2α and β -EP and the total effective rate to assess curative effect. We compared the overall outcome in both groups before and at1,2 ,and 3 menstrual cycles after treatment to find out a way which can make the curative effect last longer. Results： Following treatment, we obtained the following results in treatment group and control group ： （ 1 ） VAS score of two groups fall in 3 menstrual cycles after treatment but the variety of treatment group was more obvious than that of control group （ P 〈 0. 05 ）. （ 2 ） Two groups can make the levels of PGF2α and α -EP varied but there was no difference with t test（ P 〉 0.05 ） between the two groups,indicating their incomparability. （ 3 ） The total effective rate was 90.0% in treatment group and 80% in control group. Conclusion：（ 1 ） Two groups can make the levels of PGF2α and β - EP varied but there was no difference between the two groups. （2） Electroaeupuneture and massage can get better curative effect.
