石氏颅脑方结合高压氧早期干预防治颅脑外伤后眩晕的临床研究

摘　　要：目的:探讨石氏颅脑方结合高压氧早期干预防治颅脑外伤后眩晕的疗效及其机制。方法:94例颅脑外伤眩晕患者(伤后2周以内),随机分为综合治疗组(n=47)和高压氧治疗组(n=47)。其中高压氧治疗组患者仅接受高压氧治疗,综合治疗组在高压氧治疗同时服用石氏颅脑方汤剂。4周后比较两组患者的眩晕量表评分、基底动脉平均血流速度以及血浆黏度、脂蛋白(α);半年后,对患者眩晕量表评分进行随访。结果:与高压氧治疗组相比,综合治疗组的眩晕量表评分明显升高(P<0.05),血浆黏度和脂蛋白(α)明显降低(P<0.05和P<0.05),基底动脉平均血流速度没有改善(P>0.05)。半年后随访显示:综合治疗组能明显改善眩晕量表评分(P<0.01)。结论:石氏颅脑方结合高压氧治疗颅脑外伤后眩晕疗效肯定,其机制与中药降低血浆黏度、脂蛋白(α)有关;对于改善颅脑外伤后眩晕及其伴发症状有更好的远期疗效与预后,即有一定的防治作用。Objective：To observe the effect of treating vertigo of early traumatic brain injury with Chinese herbal compounds of Shi＇s traumatology and hyperbaric oxygen. Methods：94 patients of vertigo of early traumatic brain injury （within 2 weeks） were randomly divided into two groups： 47 cases in the combined therapy group and 47 cases in the hyperbaric oxygen （HBO） therapy group. The former with Chinese herbal compounds of Shi＇s traumatology and HBO ,the latter only with HBO. After 4 weeks ,vertigo scale scores, basilar artery flow velocity, blood viscosity and lipids were measured. Half a year later, vertigo scale scores on patients were followed up. Results：Compared with the HBO therapy group, the combined therapy group increased vertigo scale scores（ P 〈 0.05 ） , reduced indexes of the plasma viscosity and lipoprotein （tx） （ P 〈 0.05 ） without improvement of the basilar artery mean flow velocity. Half a year later,the combined therapy can significantly improve vertigo scale scores （P 〈 0.01 ）. Conclusion：With both Chinese herbal compounds of Shi＇s traumatology and hyperbaric oxygen, we can get an exact curative effect of vertigo of early traumatic brain injury,owing to Chinese herbal compounds＇ reduction of the plasma viscosity and lipoprotein （ct）. To some de- grees, the combined therapy can prevent and treat vertigo of traumatic brain injury.
