摘 要:目的探讨柳州市手足口病重症化的危险因素。方法回顾性地收集2012年1月1日~2013年12月31日在柳州市工人医院住院治疗的72例手足口病重症病例和354例普通病例,进行非匹配的病例对照研究,采用SPSS 19.0软件进行Logistic回归分析。结果单因素Logistic回归分析显示初诊单位、初诊诊断、发病就诊间隔天数、体温、持续发热天数、精神、抽搐、呕吐和血糖等9个影响因素与手足口病重症化显著相关;多因素Logistic回归结果表明发病就诊间隔天数≥3 d、体温≥38.5℃、持续发热天数≥3 d、精神差、抽搐、呕吐和血糖≥6.11mmol/L为手足口病重症化的危险因素。结论临床医生应密切关注手足口病患儿就诊史、体温、血糖等变动,尽早识别手足口病重症化的危险因素,采取有效干预措施减缓该病重症化进程。Objective To explore the risk factors of severe hand-foot-mouth disease. Methods 72 severe and 354 usual hand-foot-mouth disease cases were retrospectively investigated by case-control study. The collected data was an- alyzed using logistic regression by SPSS 19.0. Results The results of univariate Logistic Regression showed that the first visited hospital, first diagnosis, the interval of days from onset to diagnosis, temperature, mental condition, the days of fe- ver, convulsion, vomiting and blood glucose were associated with severe hand-foot-mouth disease. While the interval of days from onset to diagnosis ,the temperature higher than 38.5 ℃, a fever with more than 3 days, bad mental condition, convulsion, vomiting and blood glucose higher than 6.11 mmol/L were observed as risk factors of severe hand-foot-mouth disease by multiple Logistic regression. Conclusions Clinical doctors should pay more attention to risk factors of severe hand-foot-mouth disease, thus effective measures will be taken to prevent the disease deterioration.
