西渡社区高血压签约管理模式效果分析

摘　　要：目的 :评价高血压签约管理模式在西渡社区实施效果,探索符合西渡社区特点的高血压管理模式。方法:对500例高血压患者进行签约管理,12个月后评价效果。结果 :签约管理后的血压控制率明显优于签约管理前;签约管理后患者自我管理采纳率均有不同程度的提高,其中提高最大的为定期监测血压、规律运动和规范服药(P<0.05)。结论:签约管理模式提高了高血压患者的血压控制率,也改变了不良生活方式。Objective： To evaluate the implementation result of “the contract management” mode of hypertension in Xidu Community to explore a management mode that conforms to the characteristics of Xidu Community. Methods：Five hundred hypertensive patients were managed with the contract and their effect was evaluated after 12 months. Results：The blood control rate after the contract management was much better than that before one. The adoption rate of the self-management of the patients was enhanced different degrees after the patients adopted the contract management and the biggest growth was the regular monitoring blood pressure, regular exercise and standardized medication （P〈0.05）. Conclusion：The contract management mode enhances the blood pressure control rate, and also improves the bad life style.
