摘　　要：目的 应用经会阴实时三维超声方法评估经阴道分娩和选择性剖宫产对产后女性前腔室结构的影响.方法 以中山大学附属第三医院2012年3月至2014年4月的133例产后妇女为研究对象,于产后6～8周应用经会阴实时三维超声对经阴道分娩组与剖宫产组受检者的静息状态下和最大Valsalva动作后的各项指标进行观察与测量,包括静息状态下的膀胱逼尿肌厚度（DWT）、膀胱颈位置;最大Valsalva动作后的膀胱颈移动度（BND）、尿道旋转角、尿道内口漏斗有无形成,膀胱有无膨出.结果 两组最大Valsalva动作后的膀胱颈移动度、尿道旋转角度增大;尿道内口漏斗形成率、膀胱膨出率增高,差异均有统计学意义（P＜0.05）;而静息状态的DWT和膀胱颈位置差异均无统计学意义（P＞0.05）;结论 阴道分娩对女性前腔室的近期影响较选择性剖宫产大.经会阴实时三维超声评估最大Valsalva动作后前腔室结构的参数变化可用于来评估妊娠及分娩方式对前腔室结构的影响.Objective To evaluate the effect of vaginal delivery and selective caesarean section on female anteriorcompartmentusing the transperineal real time three-dimensional ultrasound.Methods 133 women underwent the transperineal real time three-dimensional ultrasound in 6-8 weeks after childbirth.From March 2012 to April 2014 in the Third Affiliated Hospital of Sun Yat-sen University.We observed and measured the parameters of resting and Valsalva condition respectively,including bladder detrusor thickness （DWT）,bladder neck position,the bladder neck descent （BND）,urethral rotation angle; and the existence of bladder neck funneling and vesicocele.Results The BND andurethral rotation angle after Valsalva action in the vaginal delivery group was larger than the cesarean section group; The rates of bladder neck funneling and vesicocele were higher than the vaginal delivery group,the differences were statistically significant （P〈0.05）; while there were no significant differences between DWT and bladder neck position in resting state （P〉0.05）.Conclusion Vaginal delivery is more influential than selective cesarean section on anterior compartment of female pelvic structure.Parameters of anterior compartment after Valsalva action can be used to assess the impact of delivery mode by transperineal three-dimensional ultrasound.
