血清降钙素原和C反应蛋白对下呼吸道感染的诊断价值

摘　　要：目的 探讨血清降钙素原（PCT）和C反应蛋白（CRP）在下呼吸道感染疾病早期诊断中的价值.方法 对200例下呼吸道感染性疾病患者血清中的PCT和CRP浓度进行连续检测.结果 细菌感染组90例患者治疗前PCT、CRP测定值分别为（4.23 ±3.48） μg/L、（62.38±17.13） mg/L;治疗后PCT、CRP测定值分别为（1.90±0.90） μg/L、（9.55±3.52） mg/L,治疗前、后比较差异有统计学意义（t值为6.15、29.96,P均＜0.01）.治疗前细菌感染组PCT和CRP与病毒感染组、非感染组比较差异有统计学意义（F值为41.04、12.89,P＜0.01）.结论 血清PCT和CRP在下呼吸道感染性疾病早期鉴别诊断中有重要价值,对其进行动态检测对于疗效判断有一定的临床意义与价值.Objective To explore the early diagnosis value of serum procalcitonin （PCT） and Creactive protein （CRP） on lower respiratory tract diseases.Methods Two hundred patients with lower respiratory tract diseases were selected as our subjects.Serum PCT and CRP levels were measured and analyzed before and after treatment.Results Serum PCT of 90 patients with bacterium infection before and after treatment were （4.23 ± 3.48） μg/L and （1.90 ± 0.90） μg/L respectively; and serum CRP were （62.38 ± 17.13） mg/L and （9.55 ±3.52） mg/L.Serum PCT and CRP of the patients with bacterium infection after treatment were statistically different from that before treatment （t =6.149,29.96 ; P 〈 0.01）.Serum PCT and CRP of patients with bacterium infection was statistically different from those of patients with virus infection or no infection（F =41.04,12.89 ;P 〈 0.01）.Conclusion Serum PCT and C-reactive protein show significant value in early diagnosis in lower respiratory tract infection.Dynamically testing them will help to assessment of curative efficacy.
