不同针刺强度治疗功能性消化不良疗效观察

摘　　要：目的：观察不同针刺强度对功能性消化不良（FD）的临床疗效差异。方法：将120例功能性消化不良患者随机分为弱刺激组、中刺激组和强刺激组各40例,分别在足三里、中脘、内关三穴进行不同针刺强度的治疗,治疗4周。结果：强刺激组总有效率为70.00%（95%CI=55.80%～84.20%）;中刺激组总有效率90.00%（95%CI=75.98%～97.18%）;弱刺激组为52.50%（95%CI=37.02%～67.48%）;3组综合疗效比较（χ^2=11.766 2,P〈0.01）,差异有显著性意义;B组与C组比较（｜R2-R3｜=0.221 3〉0.160 2,P〈0.05）;总体症状积分参数比较,B组优于C组（P〈0.01）;主症状积分参数比较,B组优于A组、C组（P〈0.01）;胃动素参数比较,B组优于A组、C组（P〈0.01）。结论：中等刺激强度优于强刺激组的证据尚不够充分,而优于弱刺激的证据明显,其收益为OR=0.12（95%CI=0.04～0.41）,NNT=3（95%CI=1.74～5.66）。Objective： To observe the differences of clinical efficacy treating the functional dyspepsia（FD）with different intensity of acupuncture.Methods： 120 patients with functional dyspepsia were randomly divided into weak acupuncture group,medium acupuncture group and strong acupuncture group,with 40 patients in each group,which were treated respectively with different intensity of acupuncture at three acupoints of zu shanli,zhong wan and nei guan for 4 weeks.Results ： The total effective rate of strong acupuncture group was 70.00%（95% CI= 55.80%～ 84.20%）; medium acupuncture group 90.00%（95% CI=75.98%～ 97.18%）; weak acupuncture group 52.50%（95% CI=37.02%～ 67.48%）; comprehensively compared the efficacies of three groups（χ^2= 11.766 2,P〈0.01）,the differences had marked significance; compared group B with group C（｜ R2-R3｜= 0.221 3〉 0.160 2,P〈0.05）; compared the parameters of overall symptom scores,group B was better than group C（P〈0.01）; compaerd the parameters of main symptom scores,group B was better than group A,group C（P〈0.01）; compared the motilin parameters ,group B was better than group A,group C（P〈0.01）.Conclusions： The evidence that medium intensity of acupuncture is better than strong acupuncture group is still insufficient,but the evidence that it is better than weak acupuncture is obvious,its benefits are OR=0.12（95% CI=0.04～ 0.41）,NNT=3（95% CI=1.74～ 5.66）.
