摘 要:目的探讨微量白蛋白尿(MAU)表达与老年高血压患者靶器官损害的关系。方法选择177例原发性高血压患者,统计分析其微量白蛋白(MAU)、肾小球滤过率(GFR)、收缩压(SBP)、舒张压(DBP)、总胆固醇(TC)、甘油三酯(TG)、低密度脂蛋白(LDL)、高密度脂蛋白(HDL)、空腹血糖(FBG)、2小时血糖(2HPG)、左室质量指数(LVMI)、血肌酐(CCr)、尿蛋白定性(PRO)数据,根据MAU表达分为MAU+组与MAU-组,比较两组心脑肾靶器官损害情况。结果 177例老年高血压患者中,MAU+组32例(18.08%),MAU-组145例(71.92%)。MAU+组SBP、TG、TC、HDL、2HPG、LVMI、PRO明显高于MAU-组(P<0.05);MAU+组发生心力衰竭、急性冠脉综合征、脑卒中(31.25%、40.63%、43.75%)均明显高于MAU-组;微量白蛋白尿与心力衰竭、急性冠脉综合征、脑卒中、肾功能损害呈正相关(OR=3.031、4.156、2.516、3.456)。结论微量白蛋白尿与老年高血压患者心脑肾靶器官损害明显相关,在治疗高血压疾病的同时,应关注微量白蛋白尿变化,以延缓或阻止对心脑肾靶器官的损害。Objective To study correlation of microalbuminuria and damage of target organ in elderly hypertension patients.Methods choosed 177 patients with primary hypertension,microalbuminuria (MAU),glomerular filtration rate (GFR),systolic blood pressure (SBP),diastolic blood pressure (DBP),total cholesterol (TC),triglyceride (TG), LDL (low-density lipoprotein),HDL (high density lipoprotein),fasting blood glucose (FBG),2 hours blood glucose (2 HPG),left ventricular mass index (LVMI),serum creatinine (CCr),qualitative urine protein (PRO)and were com-pared,according MAU level divided into the MAU+ group and MAU- group,damage of target organ between two groups were compared.Results 177 cases elderly patients with hypertension,MAU+group 32 cases (18.08%),MAU- group 145 cases (71.92%).MAU + group SBP,TG,TC,HDL,2 HPG,LVMI,PRO was significantly higher than MAU-group (P&lt;0.05);MAU + group heart failure,acute coronary syndrome,stroke (31.25%,40.63%,43.75%) were significantly higher than MAU-group;Microalbuminuria and heart failure,acute coronary syndrome,stroke,renal impairment were positively correlated (OR=3.031,4.156,2.516,3.456).Conclusion Microalbuminuria and damage of target organ of heart,brain,kidney of elderly patients with hypertension significantly related,should focus on mi-croalbuminuria change,to delay or prevent damage of target organ.
