多普勒超声引导下痔动脉结扎术治疗痔病的应用研究

摘　　要：目的探讨多普勒超声引导下痔动脉结扎术治疗痔病的临床效果。方法 150例内痔及混合痔患者分为观察组与对照组各75例,对照组给予传统的外剥内扎术治疗,观察组给予多普勒超声引导下痔动脉结扎术治疗,比较两组患者的治疗效果。结果两组脱出症状大部分消失,消失率无统计学意义(P>0.05);观察组术后首次排便、术后一周便血发生率均低于对照组,差异有统计学意义(P<0.05);观察组患者术后疼痛及水肿评分显著低于对照组(P<0.05);观察组有肛门下坠感及继发性出血的患者所占比例、尿潴留评分等与对照组比较差异不显著(P>0.05)。结论采用多普勒超声引导下痔动脉结扎术治疗痔病,操作简单,疗效确切,并发症少,术后恢复快。Objective To understand the clinical effect of Doppler guided hemorrhoidal artery ligation（DG-HAL） in the treatment of hemorrhoids. Method 150 cases of hemorrhoids and mixed hemorrhoid patients were divided into an experimental group（n = 75） and a control group（n = 75）. The control group were treated with traditional external ligation stripping while the experimental group were treated with Doppler guided hemorrhoidal artery ligation. The therapeutic effects as observed in the two groups were compared. Findings Prolapse symptoms of two groups almost disappeared. The disappearance rate was not statistically significant（ χ2= 4. 879,P〉 0. 05）. The improvement degree of hemorrhoidal bleeding in the experimental group was better than that in the control group. The differences in the first defecating time and one week after operation between the two groups were significant（P〈 0. 05）. Postoperative pain and edema scores of the experimental group were significantly lower than that of the control group（P〈 0. 05）. The proportion of patients who had a sense of falling anus and secondary occurrence of anal bleeding,and urinary retention score of the experimental group had no significant difference as compared with that of the control group（P〉 0. 05）. Conclusion Doppler guided hemorrhoidal artery ligation（ DG-HAL） in the treatment of hemorrhoids has the advantages of simple operation,less pain,faster recovery,and fewer complications.
